Responsibilities

Provide direct patient care in accordance with facility policies and procedures, including administering medications, wound care, and vital sign monitoring
Develop and implement individualized care plans in collaboration with the healthcare team
Conduct routine assessments and document patient information accurately and thoroughly
Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination
Participate in interdisciplinary team meetings to discuss patient progress and develop effective care plans

Requirements

Current LPN license in the state of Florida
High school diploma or equivalent required; associate's degree or higher preferred
Ability to lift, transfer, and reposition patients as needed

Ability to work varying shifts, including days, evenings, and weekends
