Job Responsibilities

  • Working as an LPN supporting the diverse needs of residents in post-acute facilities
  • Monitoring residents' mental and physical conditions and vital signs
  • Facilitating the intake process, including the documentation of medical histories and any health issues
  • Assisting with medication administration, including the verification process and the monitoring of effects on residents
  • Maintaining residents' medical records and charts
  • Helping with immunizations, IVs, and blood draws
  • Ensuring residents have a comfortable and safe environment
  • Being accountable to adhere to residents' care plans and to comply with all applicable rules and regulations
  • Contributing to the maintenance and proper inventory of medical supplies

Qualifications and Skills

  • Current New Hampshire state license to practice as a Licensed Practical Nurse (LPN)
  • Completion of approved Licensed Practical Nurse (LPN) educational program
  • Graduation from high school (or GED/equivalent)
  • At least 6 months experience working as a LPN (or similar role)
  • Ability to lift at least 50 lbs

Community Care Manager (RN or LPN)

Kendal at Hanover is a non-profit Continuing Care Retirement Community and home to 400 residents. Kendal at Hanover is seeking a Community Care Manager (RN or LPN) to provide support and oversight for resident care coordination, physical and psychological wellbeing, and transitions in care for residents across the Kendal community. The primary focus of this role is ensuring residents are residing in the appropriate level of care and collaborating with them and their interdisciplinary team when transitions are needed, including short-stays, respite stays, direct admissions, and permanent transitions to higher levels of care. As one of the Nurse Supervisors on duty, this role also provides supervision and support to nursing staff and may assume direct nursing care duties including medication administration and wound care. Essential Duties and Responsibilities include: Care Coordination, including: identifying clinical needs and coordinating services and placement; conducting care plan and family meetings; developing initial care plans; and completing CARES tools and admission assessments Resident Development and Community Outreach, including: establishing close relationships with residents and serving as primary contact for health and wellness needs; engaging with residents during wellness visits; Documentation and Compliance, including: documenting daily work with residents in the medical record; creating reminders as needed for identified gaps in documentation; keeping up to date on NH state laws and ensuring efficient surveys; Clinical and Supervisory Support, including providing clinical oversight and supervision to nursing staff; providing timely feedback for evaluations and mentoring; offering guidance and training to clinical staff during orientation; administering medication and providing treatments. Education and Experience Requirements In order to be considered for this role, an individual must: Hold current LPN or RN licensure in the State of New Hampshire and be in good standing without provisions; Have a minimum of two (2) years of clinical nursing experience with a preference for experience in post-acute care settings, geriatrics, or care management; Have demonstrated knowledge of appropriate skills for communicating with all ages, especially the geriatric population; and Be able to walk, stand, and bend most of the shit, and be able to lift up to 50 pounds. The hours for this position are Monday through Friday from 8:00 AM - 4:30 PM.
