Job Responsibilities

Working in post-acute facilities in the area as an LPN
Being accountable to follow and adhere to resident care plans
Tracking the wellbeing of residents, both physical and mental, and monitoring their vital signs
Conducting intakes, documenting health concerns and medical history of residents
Administering and properly verifying medications and monitoring their effects
Reviewing residents' charts and medical records
Assisting with the cultivation of a safe, comfortable facility
Maintaining appropriate stocks of medical supplies
Participating in IV set up, immunizations, and phlebotomy procedures

JOB SUMMARY Responsible to ensure that the MDS Department maintains a current care plan and MDS for all residents and that all documents pertaining to plan of care are consistent in accordance with current Federal, State, and local standards, guidelines and regulations, facility policies and as may be directed by the Administrator. The MDS Manager provides guidance to the interdisciplinary team in terms of MDS related reimbursement and provides support for both the nursing and therapy departments in relation to Medicare Compliance. QUALIFICATIONS/PHYSICAL REQUIREMENTS Must possess current CPR certificate or become certified within two weeks of employment. Working knowledge of OBRA, Medicare, IDPH and IDPA standards and methods of documenting according to those standards. Must be able to read, write and speak the English language in an understandable manner. Must possess the ability to deal tactfully with residents, family members, visitors, personnel, government agencies/personnel, and the general public. Must possess leadership ability and willingness to work harmoniously with personnel. Must possess computer skills. PHYSICAL REQUIREMENTS Must be able to move intermittently throughout the workday. Must be able to cope with the mental and emotional stress of the position. Must possess sight/hearing senses or use prosthetics that will enable these senses to function adequately so that the requirements of the position can be fully met. Oversees the department for the timely completion of all MDS related documents. Participates in completing MDS documents as needed to ensure compliance. Reviews validation reports and addresses any concerns with the MDS department. Chairs resident care plan conference and relays all concerns from resident and/or family/responsible party to the Administrator. Monitors ADL tracking tools in coordination with the Restorative Department for accuracy and compliance. Provides education and training in coordination with the Restorative department for the line staff on coding of the ADL tracking tools. Provides guidance for reimbursement related items to the Restorative Department. Assists the Administrator in meeting all goals and completing all projects related to MDS based reimbursement. Conducts Medicare meetings in coordination with the facility Administrator and completes regular reviews of therapy schedules and needs for COT assessments. Provides assistance with the review of the resident services, makes recommendations regarding treatment, performs documentation reviews, and other areas that will assist with maximizing reimbursement for services provided. Oversee that residents/ families/responsible parties are contacted in relation to participation in care plan conferences per RAI guidelines. Plan, conduct, and participate in In-service Education Program as it pertains to the RAI process for the facility IDT members. Assist in implementing and maintaining an effective orientation program that orients the new employee to the RAI process and data entry. Attend and participate in continuing education programs. Monitor resident’s medical record, including MDS, Cardex, Care Plans, progress notes, Physician Order Sheets, Flow records, monthly nursing summaries, IDT notes/assessments, etc. for consistency and content. Address discrepancies in documentation which are not consistent and/or do not meet program standards to appropriate department manager and Administrator. Maintain ICD-9 codes for current diagnosis. Report IT concerns to help desk timely. Participate in the development, maintenance, and implementation of the facility’s Quality Improvement program. Completes assigned or self-initiated Quality Improvement projects to monitor the Managed Care and Medicare reimbursement. Works closely with AMS MDS Coordinator on identification of problems and problem-solving action plans related to the MDS processes. Attend relevant meetings as directed by the Administrator.
