Required to support and follow the Activity Program’s mission, philosophy, policies and procedures and state and federal regulations. ESSENTIAL FUNCTIONS Provide feedback to Activity Director for monitoring achievement of facility and program goals through a team approach. Follow all Activity Policies and Procedures and Federal and State regulations. Become knowledgeable of each individual resident’s background, cultural, life history, disease, and medical care needs in detail to ensure an appropriate person-centered activity plan. Activities shall be daily and shall reflect the schedules, choices, and rights of the residents. Be able to understand communication needs of residents, such as language services, communication boards, etc. Per direction of the Activity Director, conduct, oversee, and complete initial and all on-going assessments, including but not limited to social, medical, cognitive, communication abilities, spiritual needs, physical, neuro-psychological, behavioral, amongst others as it relates to individual activity/leisure. Document and complete all required documentation per Activity Director. Maintain the following records: Resident Daily Participation, Resident Progress notes, Leisure Interest Survey, and Activity Planning sheets to assist in scheduling and meeting resident needs. Assist to update activity boards through direction of the Activity Director, with a monthly theme that is creative, neat, and clean. Facilitate community involvement, such as inviting the community to events and programs. Assist in resident outings, which can include but not limited to shopping centers, movies, restaurants, community events, etc. that may also entail driving residents to and from the resident outing. Follow Activity staff schedules with a team approach and continuity of care. Work as team with staff and volunteers. Must maintain structure of activity schedule and conduct all activities on-time; May not cancel an activity without permission of Activity Director. Must maintain equipment and supplies in good clean order. Must notify Activity Director within 48 hours if any new materials are needed for scheduled activities and when repairs are needed. Must update calendars and boards daily. Perform other duties as assigned. ESSENTIAL FUNCTIONS Ensure that all nursing procedures and protocols are followed in accordance with established policies, including dress code. Provides assistance with activities of daily living to a specific number of residents and/or as directed by the staff nurse. Reports all concerns, complaints made by the customer or their family to the Administrator. Makes rounds to assure customers are safe and comfortable. Maintains an atmosphere of warmth, personal interest, and positive emphasis, as well as a calm environment throughout the department. Participates in In-service Education Programs. Assists in implementing and maintaining an effective orientation program that orients the new Ladies and Gentlemen of Alden to the facility, his/her department, and its policies and procedures. Recommends to the Director of Nursing the equipment and supply needs of the department. Inventories and stores the new customer’s personal possessions. Assists customers with packing personal possessions when they are transferred to a new room or when being discharged. Participates in and receives the nursing report upon reporting for duty. Maintains intake and output records as instructed. Checks and reports bowel movements and character of stools. Makes occupied and unoccupied beds, changes bed linens, and straightens rooms, closet, and drawers. Notifies housekeeping if mattress in need of cleaning or replacement. Answers call lights promptly, answer telephone calls as needed. Obtain heights, and weights as needed. Does complete check of skin condition when dressing, undressing, turning/repositioning, etc. Reports any skin abnormalities to the Staff Nurse. Observes customer’s physical condition, attitude, reactions, appetite, etc., and reports any changes and/or unusual findings to the Nurse/RCC so care plan can be updated. Performs personal services for customer’s (i.e., reading, writing letters, etc.) as time allows. Encourages customers to help themselves and promotes self-care in aiding rehabilitation. Assists with care of dying and expired customers. Records resident food/fluid intake. Completes meal monitor form as indicated. Reports changes in resident’s eating habits to the Staff Nurse. Keeps customers water pitchers clean and filled with fresh water on each shift unless contraindicated. Serves between meals and bedtime snacks. Wears transfer belt at all times and utilizes as needed. Keeps the nurses’ call system within easy reach of the resident. Reports all hazardous equipment and conditions to the Staff Nurse immediately. Participates in the development, maintenance, and implementation of the facility’s Quality Assurance program for the Nursing Department. Performs other related duties as assigned.

JOB SUMMARY The Memory Care Aide assists the Memory Care Assistant Director in providing a holistic, therapeutic, person-centered Memory Care Program on the Memory Care Unit. The Memory Care Aide will achieve and maintain the highest quality of life for each resident with a diagnosis of a disease that causes dementia residing in or under the care of the facility. Additionally, the Memory Care Aide will continuously provide the evidenced-based, therapeutic programs and activities that the memory care residents engage in daily. QUALIFICATIONS Hold a high school diploma or equivalent. Have at least one year of experience in the healthcare field either working or volunteering. Have at least three months of experience engaging with persons with Alzheimer’s disease and other dementias. Must have the sincere desire to work with the memory care population. Have confidence to effectively express the Memory Care Program to the community, other health care professionals, outside professional organizations, senior services, any other provider or professional in the community and potential families and residents. Possess confidence and the ability to apply that knowledge to deal effectively with this population. Have the willingness to continuously learn about the disease process of dementia. Possess the ability to utilize therapeutic approaches effective with the dementia population on an ongoing basis. Have the ability to make independent decisions in the best interest of residents, staff, and families. ESSENTIAL DUTIES Commit to continued growth as a knowledgeable and experienced healthcare provider in the area of Alzheimer’s dementia and other diseases that cause dementia. Commit to obtaining at least 12 hours of continual education annually for self, including the required 16 hours of orientation training and any other ongoing dementia care specific trainings for direct care staff. Assist in conducting initial and all on-going comprehensive activity assessments and MDS/CAA assessments, including, but not limited to, the following areas: social, spiritual, level of functioning, communication, activity/leisure, amongst others. The information gathered will be used to create a holistic, therapeutic, person-centered memory care program. Assess, create, implement, and maintain any communication needs of residents, such as language services, communication boards, assistive devices, etc. Educate residents, family and staff utilizing these mechanisms to provide ample communication. Become knowledgeable of each individual resident’s background, life history, disease, medical and behavioral care needs, and preferences in detail to develop and ensure an appropriate person-centered plan of care. Assist in providing a therapeutic, person-centered memory care activity program that constantly strives to enhance the quality of life of each resident, meets the Memory Care Program Mission and Philosophy, and upholds the Memory Care policies and procedures and all state and federal standards. Provide both group and 1:1 activity programming. Complete all required activity participation documentation. Encourage family involvement through volunteering and visitation. Participate in monthly Family Council Meetings as appropriate. Follow-up timely on any concerns brought forth. Provide a clean, safe, non-hazardous unit for the residents and staff. Assist in providing an individualized symptom management tracking system as needed per resident, including development and maintenance of individualized, non-pharmacological interventions implemented. Assist in providing psycho-social programming in conjunction with the Memory Care group and 1:1 programming, as assessed as appropriate for residents. Assist with involvement in Accidents and Incidents process, in conjunction with the inter-disciplinary team to determine root cause, develop processes, and/or develop care plans that will decrease accident/incident occurrences. Complete all required documentation. Maintain all program policies and procedures and state and federal regulations. Provide feedback to the Memory Care Director for monitoring achievement of the facility and Memory Care Program goals through a team approach. Be flexible to meet the change in residents’ needs. We need to adapt to their needs rather than expecting them to adapt to our needs. Provide emotional, caring support to residents, families, and the inter-disciplinary team.
