JOB SUMMARY Responsible for carrying out and documenting the activities of the Restorative Program in accordance with current federal, state, and local standards, guidelines and regulations, facility policies. The objective is to ensure the highest degree of quality care is maintained at all times. QUALIFICATIONS Successful completion of a nursing arts course in an accredited nurse training program as evidenced by a diploma, certificate, or other written verification from the school and successful completion of the Department approved nursing assistant competency evaluation or successful completion of a United States military training program that includes the content of the Basic Nursing Assistant Training Program High school education or equivalent. Must possess current CPR certificate or become certified within two weeks of employment. Demonstrate interest and/or knowledge in restorative, ADL, splint, and ROM programs. Must be able to read, write, and speak the English language in an understandable manner. Must possess the ability to make independent decisions when circumstances warrant such actions. Must possess the ability to deal tactfully with customers, family members, visitors, personnel, government agencies/personnel, and the general public. Must possess ability and the willingness to work harmoniously with other personnel. Must personable, enthusiastic, and resourceful. PHYSICAL REQUIREMENTS Must be able to move intermittently throughout the workday. Must be able to cope with the mental and emotional stress of the position. Must possess sight/hearing senses or use prosthetics that will enable these senses to function adequately so that the requirements of this position can be fully met. Must function independently, have flexibility, personal integrity, and the ability to work effectively with the customers, family members, personnel, and support agencies. Must be in good general health and demonstrate emotional stability. Must be able to relate to and work with the ill, disabled, elderly, emotionally upset, and at times hostile people. May be required to lift, push, pull and move equipment, supplies, and customer in excess of 50 lbs. throughout the day. Is subject to exposure to infectious waste, diseases, conditions, etc., including exposure to the (HIV) AIDS and Hepatitis B viruses. ESSENTIAL FUNCTIONS Assure all restorative procedures are followed in accordance with established policies. Assist in developing a written plan of care for each customer that identifies the problems/needs of the customer. Attends and actively participates in customer care plan conference. Attend and participate in continuing education programs. Recommend to the Clinical Support Supervisor the equipment and supply needs of the department. Prepare materials and supplies needed for daily programs. Record daily participation of customers in the Restorative Program. Provides assistance with activities of daily living. Attend staff meetings, etc., as directed. Perform other related duties as assigned. Answer call lights promptly. Help check in all new residents and re-admissions, as well as assist in the transfer of residents to different rooms. Put resident's clothing away properly. Report any changes in resident's condition--e.g. eating habits, behavior, temperature, etc. to the charge nurse of the unit. Turn bedridden residents every two (2) hours or sooner as indicated. Wash, clean, and dry all incontinent residents. Assist the treatment nurse as assigned. Complete assignment report to charge nurse at the end of every shift. Be responsible for well-being and nursing care of all residents assigned to his/her unit while on duty. Take vital signs and monthly weights (with assistance) and record results in EMR. Document the following in the EMR for all assigned residents: a) vital signs b) weight c) meal monitoring d) bowl and bladder e) restorative records Detect and report situations that have a high probability of causing accidents or injuries to residents and/or staff. Assure that work/assignment areas are clean and free of hazardous conditions, i.e. spills, excess carts, etc. and that equipment, tools, supplies, etc. are properly stored while working, as well as before leaving such areas for breaks, meal times, and end of the work day. Assure that established infection control and universal precaution practices are maintained when performing nursing procedures. Follow established safety precautions when performing tasks and when using equipment and supplies. Assure that equipment is cleaned and prepared for the next shift. Assure that work schedules are followed. Maintain the confidentiality of resident information and honor his/her personal and property rights. Attend nursing department, Care Plan, and other meetings as directed by the DON or ADON. Attend and participate in in-service educational classes and on-the-job training programs as scheduled or as directed. Turn in Days Off and payroll special request forms in a timely manner. Follow established fire, disaster, safety, infection control, and evacuation policies and procedures. Perform other related duties as directed by the DON, ADON, charge nurse, or the Administrator.
