Document patient progress and communicate effectively with healthcare teams regarding changes in condition or care plans. Utilize medical terminology confidently when recording observations or communicating with medical professionals to ensure clarity and accuracy. Skills Proven experience in home health care or long-term care environments with knowledge of medical procedures and documentation practices. Strong patient observation skills to detect changes in condition promptly and accurately. Experience working with individuals with disabilities or in assisted living settings is highly valued. Report and document abnormal assessment findings or changes in physical, mental and emotional conditions to nursing staff. Assist with keeping the patient's environment clean and orderly. Minimum qualifications for the ideal future caregiver include: High School Diploma or GED State Tested Nurse Aide (STNA) Certification with the State of Ohio Nurse’s Aide Registry One year of clinical experience as an Aide in an acute, rehab or skilled nursing setting Valid Driver’s License (VDL) in Ohio and proof of $100,000/$300,000 automobile insurance coverage Basic Life Support (BLS) Certification through the American Heart Association (AHA) or the American Red Cross within orientation period Preferred qualifications for the ideal future caregiver include: Hospice and Palliative Care Nurse Assistant (CHPNA) Certification Home Care experience Physical Requirements: Must be ambulatory, able to climb stairs, pull, push, and facilitate movement of the patient. For field staff only: Able to drive to patients’ homes and/or facilities.
