What You’ll Do: Assist clients with activities of daily living (ADL's) like bathing, dressing, toileting and grooming. Provide companionship and emotional support, engaging in meaningful conversations and activities. Support clients with mobility, medication reminders, meal preparation, and light housekeeping. Create a positive, safe, and nurturing environment that promotes independence and quality of life. What We’re Looking For: 1+ year of caregiving experience (private or facility-based). Hospice experience, incontinence experience and hoyer lift experience Prefered. High school diploma or equivalent. Ability to lift or transfer up to 50 lbs. Valid driver’s license and insured vehicle. Position Summary: Under direct supervision, provides personal and homemaking services to ill, convalescing, elderly, and disabled persons in the patients' home. Essential Functions and Responsibilities: Provides personal care of a patient, including bathing, grooming/hygiene, and toileting. Takes and records daily vital signs. Reports all vital signs that are outside the normal range. Assists patients with positioning/turning, transfers, catheter care, and range of motion exercises. Light housekeeping and food preparation. Recognizes and reports any unusual, untoward, or new symptoms/changes to the RN. May perform functions to support the telehealth program (uninstalls, equipment cleaning and maintenance, installations/setups). Performs other related duties as required and directed. Required: High school diploma or equivalent. Successful completion of a written home health/hospice competency test and the clinical skills testing. Certification as Home Health Aide (CHHA). Preferred: Six months of experience with a variety of patients. Position/Subsidiary Specific: Valid driver’s license, proof of valid auto insurance, and maintain a driving record in accordance with McLaren’s corporate driving policy and eligibility guidelines.
