Travels between multiple locations to perform the duties and responsibilities of the role and is accessible to staff at all locations during working hours. Depending on the program, sometimes required to be accessible after hours for consolation. ESSENTIAL JOB FUNCTIONS To perform this job successfully, an individual must be able to satisfactorily perform each essential function listed below: Conducts the medical assessment of each individual referred to the program and collaborates with an Intake staff member to determine appropriate placement in the community Participates in the development and implementation of service plans and supervises, in conjunction with the physicians, the medical components of the plan Reviews documents and revises all nursing/nursing related care plans on a regular basis Ensures medical records and reports are current and complete in accordance with regulatory and corporate standards, including medication administration records and physician’s orders Maintains contact with primary care physicians and specialists, and, when directed, attends doctor visits with mentor and/or individual receiving services. Promotes health and wellness for individuals and ensures nursing actions and steps are put in place for prevention Administers medications according to established guidelines Provides documentation for all medications given and reviews medication administration records for compliance Oversees the management of high-risk medical conditions and private duty nursing services Develops training flow charts specific to each individual receiving services, provides and documents medical skills training to clients, mentors, respite providers, staff, and where appropriate, the biological family Presents medical skills components at Pre-service Orientation Participates as an Individual Health Plan (IHP) or Plan of Care (POC) team member, Evaluates the medical needs of each individual for initial IHP/POC, Ensures IHP/POC goals and objectives are medically indicated, and Generates ongoing health care protocols for the specific medical conditions outlined in the IHP/POC Provides consultation and technical assistance to staff and mentors regarding home care issues along with operation of durable medical equipment. May be required to coordinate, organize and/or assist with household activities such as light housekeeping and meal preparation; transport individuals receiving services to planned and/or necessary activities and appointments Participates in on call system as needed Travels between multiple locations during work hours Provides support and is accessible to staff at all locations during working hours May be required to be accessible after hours for consolation May actively participate in IDT meetings, Human Rights Committee, Health and Safety Committee and Administration meetings Performs other related duties and activities as required SUPERVISORY RESPONSIBILITIES None Minimum Knowledge and Skills required by the Job The requirements listed below are representative of the knowledge, skill, and/or abilities required to perform the job: Education and Experience: Graduate of an accredited RN program One year experience working with medically complex individuals. Certificates, Licenses, and Registrations: Current state RN License Current driver's license, car registration and auto insurance if driving on the behalf of the Company or providing transportation to individuals receiving services. Current CPR/First Aid certification. Blood Borne Pathogen Category: Job requires performance of duties that involve potential for exposure to blood, body fluids, or tissues. Tasks that do involve exposure are an expectation of employment. Physical Requirements: Heavy work. Provides training and mentorship to Infusion Nurse I’s as needed to ensure patient safety and compliance. Provides effective and safe teaching using patient-centered care approach for patient and family to achieve independence with prescribed therapy and care needs through active participation per plan of treatment goals. Effectively and timely communicates with Option Care’s Clinical team, medical providers, patients, and families/caregivers to facilitate continuity of care. Prepares clinical documentation in real-time during visits and submits to the nursing department to comply with established timelines for billing optimization. Completes all documentation legibly and applies approved abbreviations and documentation error correction practices per Option Care’s policy. Coordinates discharge planning and prepares discharge summaries with patient instructions and thoroughly reports patient care needs, progress and goals when transferring care. Demonstrates compliance with agency operations, Option Care’s policies and procedures, professional standards, local, state, federal regulations/guidelines, and accreditation standards. Maximizes work efficiency through the use of computers and other automation technologies to validate plan of treatment orders, communicates patient care provided, and follows assigned visit schedule. Responds promptly and appropriately to patient requests. Initiates emergency procedures as necessary. Accepts accountability for own practice through ethical and professional conduct. Follows established programs and practice within policies and procedures reflective of Option Care’s mission, values, and objectives. Observes legal and ethical guidelines for safeguarding the confidentiality of patient and proprietary Option Care information including adherence to HIPAA regulations. Speaks knowledgeably about Option Care’s scope of services and effectively instructs patients about related financial obligations for care and service charges. Attends and completes required training modules, in-services, and continuing education to maintain competency and professional licensure for demonstrated knowledge regarding the care and management of patients in the home and/or alternate care settings. Effectively provides oversight and coordination of paraprofessionals in the home setting and may act as the Supervisor in the absence of the Supervisor or Nurse Manager as applicable in accordance with state and federal regulations. Participates in multidisciplinary team conferences and provides precepting, training, and mentoring to other nurses for orientation, and onboarding and supervisory activities as assigned. In this role, the CC CN II will be expected to collaborate with patients to facilitate healthy behaviors through health coaching to foster healthy diet, exercise, medication, and disease management. The CC CN II helps patients learn strategies and skills designed to stabilize symptoms and disease progression through ongoing support and reinforcement of the plan of care. This role positively impacts the patient's quality of care and ability to access care and reduces unnecessary costs and capitalizing on revenue generating opportunities. Key functions include building relationships with patients, functioning as a team member in the patient's continuum of care, improving patient experience, mitigating avoidable hospital readmissions, closing care gaps, and identifying social determinant of health barriers that impact the patient's ability to maintain optimal health and wellness. Responsibilities Assists in the management of patients with chronic diseases following established protocols and systems for disease management in collaboration with providers. Assist patients with self-management of chronic disease process including patient goal setting, teach-back method of learning, and promotion of patient advocacy that extends into the wider community. Assists patients and families with coordination of healthcare services including outside organizations. Act as a client advocate partnering with Community Health Workers (CHWs) to provide assistance with social determinant of health (SDoH) needs; initiating referrals to community-based organizations as needed. Develop and evaluate a patient plan of care and determine areas of improvement and education. Monitors patient's healthcare pathway to ensure adherence, removes obstacles and identifies progress toward desired care outcomes; intervenes to overcome deviations in the expected plan of care; reviews the care plan with patients in conjunction with providers; interacts with the multi-disciplinary care team to negotiate and expedite scheduling and completion of tests, procedures, and consults. Collaborates with Care Coordination leadership to participate in and implement process improvement and quality improvement initiatives to maximize patient care outcomes. Engages physician and practice team in proactive patient management by addressing medical and behavioral health care needs, follow-up, and referrals. Utilizes high risk patient data registries, hospital and payer reports, and other reports to identify and outreach targeted populations benefiting from care coordination programs. Offers and coordinates care for complex patients in the practice setting or home as necessary to reinforce disease management education utilizing teach back methods; assist with completion of health care proxy, advanced care planning, or community resource navigation. Contributes to comprehensive care plans in collaboration with providers and the multidisciplinary health care team based on evidence-based best practices for chronic illness care. Participates in developing patient-centered care plans that address problems /barriers/goals and executes action plans relevant to obstacles in chronic condition management. Provides referrals to appropriate community resources and support programs and closes the loop by ensuring that patients can access and follow through on these referrals. Collaborates with Population Health Care Coordination leadership to engage and educate providers and teams on newest trends and optimal work patterns to improve the quality of patient care coordination. Reviews high-cost patients with the multidisciplinary care team to understand drivers of cost, current treatment plan, future course and prognosis. Ensure advance directives and appropriate referrals are addressed such as palliative/hospice and makes recommendations for cost reduction. Improves quality by assisting with closing care gaps. Reduces healthcare costs by preventing avoidable hospital admissions/readmissions and unnecessary utilization of healthcare resources. Assess and address social determinant of health barriers in collaboration with Care Coordination Community Health Workers (CHWs). Promotes patient self-management of chronic conditions. Identify and submit referrals to address patient behavioral health needs. Care coordinate high-risk patients with chronic conditions and high risk stratification scores utilizing payer, claims, PRAPARE tool, demographic, and co-morbidities data including but not limited to COPD, CHF, Diabetes, HTN, and increased acute care and ED utilization.
