Ensure compliance with Sunrise’s Timekeeping and Meal/Rest Period policies, monitor team member break schedule and duration, ensure care managers clock in/out using correct job codes, and coach team members as required. Display and encourage flexibility in work schedule by working holidays, evenings, weekends, and additional shifts when necessary. Prepare for shift by obtaining resident group assignments before care managers arrive and print and hand out to appropriate care managers. Participate in crossover meetings by sharing/receiving pertinent information with/from other Lead Care Managers. Carry a resident group assignment on overnight shifts and pick up group assignments in the event of an unfilled call off on day and evening shifts. Cross train and possess credential (including certifications where required by state/province) to safely administer medications if needed during times of high volume or unexpected absence. Lead by example when clocking in/out and taking proper meal/rest breaks. Participate in group interview process including job preview video and community tour. Support onboarding of new team members including shadowing and skills demonstration. Maintain a safe and secure environment for all staff, residents, and guests by following established safety standards and actively support safety practices. Shift Type: Per Diem Compensation: $70/visit for initial assessment and $60/visit for 60-day visits Visiting Angels is seeking a Home Care Field Registered Nurse (RN)/Case Manager for in-home visits to our clients throughout Mercer County and ideally some Burlington County clients, as well (optional). This is a non-medical per diem position There are no set hours for this position Our Home Care Field Registered Nurses (RNs) prepare thorough and accurate Initial Assessments and Reassessments and develop personalized care plans for our clients. This part-time/per diem position is ideal for an RN who desires a significant level of work but with flexible hours. Most visits to our clients occur on weekdays. ---------- Responsibilities include: Making appointments with Visiting Angels' clients in their Mercer County homes during a regular Visiting Angels' care shift; mileage is reimbursed per Company policy Creating in-home initial client assessments and care plans based on clients' specific care needs Conducting follow-up in-home assessments at specified intervals Conducting periodic telephone assessments Revising care plans as necessary Supervising Certified Home Health Aides (CHHAs) Performing Orientations/Skills Assessments of CHHAs ---------- Qualifications include: Previous Home Care RN experience HIGHLY preferred Current New Jersey Registered Nurse (RN) license Ideal candidate resides in Mercer County Enjoys meeting and talking with older adults Prior Registered Nurse (RN) experience with in-home care assessments Excellent written and verbal communicator with strong interpersonal skills Must be self-motivated, organized, and able to work independently Able and willing to travel to clients' homes throughout Mercer and/or Burlington Counties Has a valid driver's license and a reliable car, willing to travel to clients' homes Is adept with digital/computer skills (data entry on a web-based program, email, scanning, texting) Has weekday flexibility After applying, you'll receive a link to a few simple screening questions.
