Communicate with and update the Supported Living Coah on a regular basis, if applicable. Assist the consumer with recreational, cultural and vacation plans of his/her choice. Provide independent and functional living instructions to the consumers. Complete all state required trainings. Assist with shopping (e.g., food, clothes). Education and Experience : High School Diploma or G.E.D. Required, copy. Must have valid driver’s license and a clean driving record. Must be able to work independently. Must have 1 year of experience with Disabled Children/Adult, Nursing Home or Home Care.
