Job Summary GENERAL SUMMARY We are seeking a highly skilled, compassionate Registered Nurse (RN) to deliver high-quality, patient-centered care through a combination of in-home patient visits, telehealth patient consults with Nurse Practitioners or Physicians as required, and administrative tasks. The CareConnect Team at WellBe supports timely response to patients’ changing needs and this role will be instrumental in delivering this care consistently and in a patient-first manner. This role primarily supports urgent/episodic interventions, working in partnership with physicians, nurse practitioners (APPs), and interdisciplinary care teams to address a range of patient needs across the care continuum. The RN will operate with a patient-first mindset, balancing strong clinical judgment with care coordination, education, and documentation responsibilities. The role has no direct reporting responsibility but will function as part of an integrated, interdisciplinary patient care team. Maintain accurate, timely, and complete patient records, ensuring documentation is updated at each patient encounter. Attend and actively participate in required training programs, including medication administration, clinical equipment use, new protocols, and ongoing professional development opportunities. Build and maintain positive relationships with local healthcare providers, community organizations, caregivers, and referral sources to support WellBe's presence and reputation within the community. Utilize available internal and external resources to address patient needs, including behavioral health services, community-based programs, and other supportive care resources. Maintain all required active, unrestricted state licensure and certifications. Adhere to professional standards of practice, ethical conduct, patient confidentiality requirements, and organizational policies.
