We provide comprehensive medical care, such as infusion therapy and wound care, for children with complex medical needs. Other team members include nurse Liaisons, physical, occupational, speech and neonatal pediatric respiratory therapists, as well as experts in pharmacy, medical social work and durable medical equipment and supplies. Using a multidisciplinary approach, we actively coordinate the patient’s individualized plan of care. We provide short-term medical care, education, encouragement, and support to help our patients and families attain the highest level of health and independence. The Children’s Mercy Home Care Team teaches the child’s parents or legal guardians how to independently provide the care ordered. Our goal is to prevent hospital readmissions and ensure the best possible outcomes. Our Home Health Nurse Specialists provide comprehensive medical care and perform a wide range of skilled nursing services including acute and rehabilitative care, assessment of high-risk newborns, assessment and teaching of various systems, diabetes and asthma teaching, wound care, infusion therapies and central line care, wellness and/or disease-focused education, and other specialized treatments. We see patients from birth to 18 years of age for a broad range of conditions including pre- and post- transplant care, multi-congenital abnormalities, cancer care, wound care, failure to thrive and feeding difficulties, and preterm newborns to name a few. Our skilled nursing visits typically last around one hour and our nurses see several patients a day. Our Home Health Nurse Specialists see patients in the comfort of their own home, at school or daycare, or a relative’s house. Our Home Health Nurse Specialists have a lot of autonomy and manage their own caseload. They enjoy the flexibility of seeing patients outside of the walls of the hospital or clinic, and in the comfort of their own home. Role Overview As a Home Health LPN/LVN, you provide direct patient care under the supervision of a Registered Nurse, helping to implement care plans and ensure patients receive safe, effective treatment in their homes. You are a key part of the care team - delivering treatments, monitoring patient conditions, and reinforcing education to support successful outcomes. What You’ll Do Provide direct patient care in accordance with the state Nurse Practice Act Implement and support the plan of care developed by the RN Deliver skilled nursing services, including treatments, medication support, and wound care Monitor patient condition and report changes to the RN and care team Educate patients and caregivers on self-care, treatments, and disease management Maintain infection control and proper use of equipment and supplies Document patient care accurately and timely within the EMR Communicate effectively with physicians, nurses, and interdisciplinary team members Participate in case conferences, team meetings, and ongoing training Support coordinated care by maintaining consistent communication across the care team What You Bring Active LPN/LVN license in the state of practice Graduate of an accredited practical/vocational nursing program Current CPR certification Minimum one (1) year nursing experience preferred (Home Health, Med-Surg, or Acute Care a plus) Strong clinical and observational skills Clear communication and patient education abilities Ability to follow care plans while recognizing and reporting changes in condition Basic proficiency with EMR systems Valid driver’s license and reliable transportation What Sets You Apart Experience in home health or community-based care Strong patient engagement and relationship-building skills Ability to work independently while staying connected to a team Familiarity with Homecare Homebase (HCHB) What to Expect Field-based role providing care in patients’ homes within an assigned territory Close collaboration with an RN and interdisciplinary team A structured yet flexible daily schedule A role focused on delivering consistent, high-quality patient care Performance expectations tied to quality, documentation, and patient experience If you’re passionate about delivering compassionate, hands-on care and want to be part of a team that truly values the work you do - VitalCaring is the place for you.
