Job Description We are seeking talented team members at Kisco Kensington. The Caregiver administers resident medication or assist residents with self-administration of medication and coordinate treatments to residents. Provide assistance with activities of daily living while encouraging independence and ensuring each resident’s physical, social, cultural, and intellectual well-being. Schedule : Part time, AM, PM, or NOC Pay: $22/$23hr What will I do every day? Observe residents for any changes in condition and report to Care Services Director, Resident Care Coordinator (RCC)/Dementia Care Coordinator (DCC), or appropriate physician Assist residents with simple requests as needed or find an appropriate associate to assist Learn resident preferences for opportunities to provide exemplary customer service, exceed resident expectations and enhance resident satisfaction Document and report changes of condition and use company forms for resident care documentation Provide medication set up and assistance Log in medications from pharmacies, verify physician’s orders and communicate any concerns immediately Track all medications and re-order as needed Perform the following tasks with residents: track vital signs, weight, diabetic testing and/or insulin injection (if state allowed), ambulation, and general activities Follow all regulatory and company policies regarding the destruction of medications as identified by the physician, pharmacy, or Care Services Director Assist in documenting and removing medications that are discontinued by the resident’s physician Familiar with the meaning and usage of common medical abbreviations, symbols, and terms relative to the administration of medications Support, lead and participate in resident-centered wellness programs Work toward maintaining each resident’s self-respect, personal dignity, and personal safety Assist resident assistants as needed and whenever primary tasks are completed Track supplies that are needed and communicate to Care Services Director or RCC/DCC Work as part of the team within your department and across the community Support open and honest communication with and between all associates Maintain high standards of personal appearance and grooming, which includes wearing the proper uniform and name tag when working Look for ways to reduce costs and create efficiencies on a daily basis Ensure regulatory compliance and report any issues or concerns immediately Support continuous improvement Adhere to all regulations, laws and Kisco standards, including the Associate Handbook Understand and support the community sales process Perform other related duties as required What will I need to be successful in this role? How you make a difference The Nurse Practitioner provides on-site medical services by examining referred patients, conducting clinics, rounds, and responding to urgent and emergency situations. The role delivers ongoing medical care by diagnosing conditions, prescribing treatment plans, managing chronic care, and providing Medication Assisted Treatment services, including induction, follow-up, and medication management. The position documents care, orders and interprets labs, adheres to established policies and formularies, and supports continuity and standards of care within the facility. Key Responsibilities Examine patients, conduct sick call, chronic care clinics, and infirmary rounds, and respond to health emergencies. Diagnose conditions, prescribe and manage treatment plans, and provide ongoing medical and MAT-related care. Conduct MAT assessments, inductions, follow-up appointments, and manage medication continuation and titration. Order and interpret laboratory tests and drug screens and utilize appropriate in-house and referral resources. Document patient encounters, maintain confidentiality, and comply with established policies, procedures, and standards of care. Qualifications & Requirements Education A graduate of an accredited school of nursing An advanced degree in an approved and accredited Nurse Practitioner program. Experience Prior medical/surgical and/or emergency/trauma experience and corrections experience is preferred. Must have the patience and tact to deal with the inmate population and ability to work effectively in the corrections environment. Scheduling flexibility is also required to be able to rotate hours and shifts, if needed, and to be called during emergency situations to provide coverage. Licenses/Certifications Must have and maintain current licensure as a Nurse Practitioner within the state of employment. Must have and maintain current CPR certification.
