A LVN Charge Nurse (Licensed Vocational Nurse) is a licensed professional nurse directly under the supervision of the Assistant Director of Nursing Services. The Charge Nurse is responsible for staff assignment and provides overall supervision of resident care activities. SUPERVISION • Meet with unit personnel, on a regular basis, to assist in identifying and correcting problem areas, and/or the improvement of services. • Review and evaluate assigned personnel and make written evaluations to the RN Supervisor or Assistant Director of Nurses. • Coordinate Nursing service with other departments and nursing units. • Notify the attending physician, RN Supervisor and Director of Nurses when a resident is discharged without approval. Assure that appropriate documentation is made in accordance with the facility’s established policies and procedures. • Assure that a stock of medications, medical supplies, equipment, etc., is maintained at all times. Requisition equipment and supply needs as required. • Initiate investigation of accidents and unusual occurrences and make the necessary written report to the Director of Nurses as established in the facility’s policies and procedures. • Notify the resident’s attending/alternate physician, as well as the resident’s legal guardian/representative, when the resident has a significant change of condition or becomes critically ill or injured, as outlined within facility’s established policies. • Work with the facility’s consultants as necessary and implement recommended changes as directed by the RN Supervisor of the Director of Nurses. • Investigate complaints and grievances and make necessary oral/written reports to the RN Supervisor or Director of Nurses. • In cases of death, notify the funeral home and ensure that the body is properly prepared/cleaned for pick-up. • Evaluate emergencies and determine emergency measures to be used, including the use of chemical or mechanical restraints. Notify the RN Supervisor and physician and record and follow orders. • Assure that the unit area is maintained in a clean and safe manner for resident comfort and convenience, by ensuring that necessary equipment and supplies are maintained to perform such duties/services. • Assist in standardizing the methods in which work will be accomplished. Illumination Health + Home is a growing non-profit organization dedicated towards disrupting the cycle of homelessness by providing targeted, interdisciplinary services in our recuperative care centers, emergency shelters, housing services and children's and family programs. IH+H currently has 13+ facilities with 22+ micro-communities scattered across Orange County, Los Angeles County and the Inland Empire. Job Description The Medical Coordinator is responsible for delivering high quality medical care and influencing positive outcomes for patients experiencing homelessness in the recuperative care setting. We have many schedules available for this role, including weekends and nights. The pay range for this position is $30.00 - $35.00 per hour, depending on experience . This position also qualifies for $1.50 swing shift differential and $3 graveyard shift differential. The schedule for this position is Thursday - Monday OR Friday - Tuesday from 3:00pm to 11:30pm. Workdays may vary. Responsibilities Client Care: Perform initial intake to ensure that an appropriate care plan is established based on discharge information from the hospital or primary care notes Assess health status of patient upon arrival by conducting head to toe assessment and checking vitals Determine the patient’s insurance status, make follow up appointments with patient’s providers and reviewing medication list Establish and maintain a care plan Set weekly goals for patient to complete to assist with medical or social reintegration, verify goal completion, and assess medical needs. Conduct morning, afternoon, and evening checks for all assigned clients to observe wellbeing, ensuring there is no change in medical conditions since hospital discharge. Link clients to medical services when necessary (insurance, PCP, specialists, referrals). Coordinate home health visits (if needed) and follow-up on medical appointments. Promote health literacy by providing patient education with regards to clients’ illnesses, medications, preventative measures, treatment management and overall general health. Arrange transportation for clients to and from medical appointments, as needed Report emergency situations to Supervisor/Manager and document incident Seek proper level of care to assess and treat medical complication Initiate medical response system when appropriate Support patient recuperation by providing necessary items and space needed to rehabilitate and assisting with the upkeep of the facility. Administrative/Record Keeping: Enter patient information and case notes into EHR and other documentation systems. Complete and update medication, vitals, progress notes and care plan Perform administrative tasks such as answering phones and filing paperwork. Prepare patient reports for billing. Maintain proper inventory of medical supplies and medications. Compliance/Safety: Maintain confidentiality in accordance with HIPAA guidelines, Illumination Foundation policies and DHS standards. Provide patients with proper compliance forms upon admission Utilize infection control measures according to OSHA standards. Conduct medication reconciliation and ensure proper disposal and pickup of bio-medical/pharmaceutical waste. Maintain a safe work environment by ensuring the cleanliness of the med room, wound care room, and medication storage room. Qualifications Required: Current and non-restricted LVN license in California 2 years of relevant experience. High School Diploma or equivalent. Basic computer skills, including the ability to send and receive emails and summarize data in spreadsheets. Valid CA Driver’s License. Preferred: Availability to work weekends and nights Home Health nursing experience and experience working with populations dealing with addiction and behavioral health conditions. Bilingual in English and Spanish. Proficiency in Microsoft Office (Word, Excel, Outlook, etc.) and KIPU or other electronic health record systems.
