Description The Clinical Program Manager is responsible for assuring compliance with mandated clinical orientation, competency assessment and interventions in the assigned clinical program of designated nursing staff within the Medical Center. Demonstrates knowledge of hospital, departmental and care center standards, policies, procedures and guidelines as well as regulatory agency standards. Assures readiness for Joint Commission and other agency surveys and presents related programs during Joint Commission and other agency visits. 6. Participates in the development and implementation of continuous quality improvement programs. 7. Participates in quality management activities including identifying and communicating issues of performance improvement and risk in a timely manner. 8. Participates in committee activities. 9. Description The Nurse Manager is responsible for the management of nursing practice and operations on a designated unit/service. This includes, but is not limited to, the management of: clinical nursing practice, staff recruitment, retention, labor relations, supply chain and budget, staffing, employee performance evaluation, quality improvement, staff education, medical records, internal and external regulatory and survey requirements. The Nurse Manager collaborates with multiple professionals to support and coordinate the provision / management of patient care. He or she works closely with the Nursing Clinical Director, unit Dyad partner and staff in insuring that the Service/Unit maintains the highest standards of patient care and is responsive to the needs of the patient & family as well as the organization Responsibilities PATIENT CARE 1. Leads nursing practice using the tenets of the Professional Practice Model of Relationship Centered Care and the Care Delivery Model of Modified Primary Nursing for the promotion of the patient experience. 2. Demonstrates proficiency in all aspects of Purposeful Hourly Rounding (PHR) and role models expected behaviors. 3. Ensures the integration of PHR into clinical activities to enhance Relationship Centered Care. 4. Validates staff members round hourly on all patients by direct observation and patient feedback. 5. Assesses staff competency in PHR and provides consistent meaningful feedback with more focused feedback as needed. 6. Conducts rounding of all patients on a daily basis (Monday-Friday.) 7. Communicates quantitative and qualitative information regarding the patient experience and staff engagement to the staff and the Interdisciplinary Patient Care Team that leads to the development and implementation of an action plan. 8. Collaborates with the physician dyad and escalates concerns as appropriate. 9. Follows up on all patient concerns in a timely manner and provides service recovery as needed. 10. Demonstrates caring, respect, compassion, empathy and active listening through dialogue, body language and actions. PATIENT EXPERIENCE 1. Role models the tenets of Mount Sinai Relationship Centered Care (MSHRCC) through facilitation of the following actions but not limited to: Admission Welcome, Bedside Shift Report, HELP (High Risk Medications, Equipment, Lines, Drains, Airway, Pain and Plan of Care), AM/PM Care, 5 Minute Sit Down, Purposeful Hourly Rounding (PHR), Medication Review, Discharge Wrap Up, and MD/RN Unit Collaboration. 2. Huddles with staff at the start of each shift, identifying high-risk patients who require special attention. 3. Communicates to nursing leadership concerns or issues identified during huddle and throughout shift. 4. Initiates service recovery for patients and family members who have concerns and escalates to Associate Director of Nursing/designee and Patient Relations as needed. 5. Interprets and communicates patient experience scores and benchmarks and keeps staff informed of progress and areas of opportunity. 6. Considers the patient’s values, preferences, cultural diversity, expressed needs and knowledge in all aspects of care. 7. Empowers staff to use evidence-based practices to increase understanding of patients’ perceptions of care. 8. Designs, supports and leads a safe, therapeutic and efficient patient-centered care environment for professional practice. 2. Identifies and investigates issues or variances in practice/operations by participating in the RCA process and implements corrective action plans. 3. Fosters a fair and open culture that encourages error and near-miss occurrence reporting. 4. Leads interdisciplinary quality improvement teams using the principles of high reliability. 5. Reviews unit progress, changes, and compliance with quality and safety metrics with clinical nurses and other members of the healthcare team. 6. Responds to escalations of potential safety hazards or gaps from best practice by correcting concerns following institutional chain of command protocol. 7. Monitors institutional information technology infrastructure and corrects gaps in clinical nurse documentation. 8. Leads staff participation in quality and safety initiatives and mandated compliance measures. 9. Demonstrates accountability for all unit quality and safety practices and compliance, and ensures required evaluation of staff competencies is maintained. OPERATIONS AND ADMINISTRATION 1. Supports implementation of the mission, vision, goals and objectives of the department, throughout the clinical service and individual patient care units. 2. Provides administrative and clinical oversight to ensure that all activity (staffing, support services, inter disciplinary collaboration, resources and financial viability) supports the goals of the organization. 3. Works collaboratively with physician dyads and all members of the healthcare team to promote patient care and regulatory standards. 4. Maintains a nursing unit in compliance with all regulatory agency mandates, including the regulations of NYS DOH and The Joint Commission. 5. Oversees 24 hour patient care delivery, throughput, staffing and processes to ensure efficient and effective operations. 6. Prepares, implements and monitors unit budgets, including capital equipment allocation, budget variances, management of OT and vacancies according to MSHS financial goals. 7. Maintains control of established positions and manages the recruitment and retention of qualified staff. 8. Maintains patient/employee confidentiality in the management of information. 9. Evaluates nursing staff performance and mentors, coaches, counsels and disciplines staff as needed. 10. Monitors availability and maintenance of nursing equipment and supplies. 11. Conducts regular staff meetings to ensure communication with staff about progress, changes and compliance with all hospital policies and procedures. 12. Encourages, recognizes and rewards staff participation in quality and safety initiatives, compliance and successes. 13. Facilitates organizational change initiatives by using effective strategies. 14. Leads a shared decision making model by establishing a structure of shared governance, just culture and staff engagement in practice councils, departmental meetings and initiatives. 15. Interprets and implements collective bargaining agreements affecting staff. 16. Description The Clinical Program Manager is responsible for the implementation and conduct of clinical trials with investigational anti-cancer agents in the Cancer Center. The Candidate must have expert knowledge of cancer as a disease process, cancer treatment modalities, and the process of conducting clinical research. The Clinical Program Manager will follow a caseload of patients in the ambulatory setting and assist in the administration and plan of the research program. This position will be primarily assigned to our Downtown Clinical Research Programs which operate at Mount Sinai West and Mount Sinai Chelsea but is expected to move throughout the health care system based on patient needs. Responsibilities Clinical Responsibilities: • Provides exemplary patient care while ensuring the protocol is conducted in a high-quality manner and clinical research documentation is inspection ready. • Responsible for coordinating study enrollment, establishing a database/case load of patients, protocol treatment and follow-up care for patients participating in clinical trials. • Responsible for ensuring protocol adherence and accurate source data. Implement the standards for research protocols in compliance with regulatory, institutional, and external agencies • Collaborates with attending physicians to identify and recruit patients eligible for clinical trial. Oversees scheduling of patient laboratory and radiologic assessments, admissions, and clinic visits and supports investigators with the informed consent process and documentation. • Evaluate, treat and/or triage untoward reactions according to the standard of care and protocol guidelines, prescribe and manage supportive care medications. • Assures the primary nursing staff understands how to administer investigational agents per specifications of the protocol. • Ensures accurate and timely research documentation and works with ancillary departments (i.e. infusion, radiology, etc.) to ensure research is integrated into standard clinical operations. We are looking for positive, dependable, caring Personal Care Aides to workon night shifts . The Personal Care Aide position requires the following: Ability to communicate in a dignified, respectful manner. Ability and interest in learning and staying current regarding medications. Specific Requirements: Must be at least 18 years of age. Prefer prior experience with medication assistance. Prefer previous experience, either professional, personal or volunteer, with the elderly population. The Personal Care Aide Duties include but are not limited to the duties listed below. Medication Assistance: Count Control Substance with outgoing PCA (sign your initials if count is correct). Report any missing controlled substances to administration. Prepare medication, bring medication to dining room during meal time, and assist residents with medication. If a resident requires medication prior to breakfast, PCA shall go to the resident's room and assist the resident with medication. If a resident requires afternoon or after dinner medication, the PCA shall assist the resident with medication from the med room. PCA must always watch residents ingest medication and properly document. PCA shall update medication administration record as needed, after Physician visits & ensure MAR & doctor's orders match PCA shall ensure that resident receives all necessary documents prior to doctor appointments. Consult and correspond with doctor offices for any necessary clarifications.. Record all pertinent information re: residents in the 24 hr. Log book. Personal Care Assistance: PCA shall assist residents in the dining room during mealtime by distributing meals to residents. PCA shall assist with cleaning the dining room after meals when required. PCA shall distribute facility provided snacks to residents during evening snack time. PCA shall assist residents with showering/bathing as needed. PCA shall assist residents with maintaining a healthy hygiene routine as needed. PCA shall assist residents by laundering residents clothing, linens, and towels as needed. PCA shall assist residents that require limited mobility assistance as needed. PCA shall answer all call bells, and check residents' whereabouts during rounds. PCA shall assist residents with operating the elevator lift. PCA shall assist residents by performing housekeeping and light maintenance tasks as needed. These tasks include but are not limited to: laundering residents clothing, linens, and towels, cleaning resident rooms, making residents’ beds, unclogging toilets, adjusting room temperature, salt areas outside of facility.
