Job Duties and Responsibilities: Provide direct patient care for Mohs patients; intake & prep patients for Mohs layers and reconstructive surgery, assist providor with surgeries, provide patient education to patients and family members, post-op care to return patients, instrument processing, patient notification, tray prep, nurse triage. Assess the environment and make recommendations to ensure optimal patient comfort, safety and compliance with various regulatory bodies; OSHA, JCAHO, etc. Complete daily assignments for the completion of indirect patient care tasks required to support direct patient care activities. These tasks include but are not limited to; telephone messaging, cleaning and packaging instruments, room supply stocking/cleaning, uploading photographs in the photo database, upcoming surgery charts, and place orders as needed. Able and ready to act as a nurse preceptor for new hires. Assist with new staff competencies and skills development as outlined in the unit specific competency based orientation checklists and policies & procedures. Participate in your own professional development by maintaining required competencies, identifying learning needs and seeking appropriate assistance or educational offerings. These items include; OESO/LMS trainings due in September, BLS, Nursing Licensure. This also includes attendance in the mandatory monthly Mohs staff meetings and nursing meetings. Cultivate exemplary customer service throughout the organization; promote effective communication skills to include, but not limited to problem solving, assertiveness, and support for team members, feedback, active listening and conflict resolution. Monitor clinic flow and identify measures to facilitate the movement of patients through all aspects of their visit. Inform HCA, NM or Nurse Team Lead of prolonged delay. Collaborate together and with the NM or Team lead to restructure assignments in the event of staff “call-out’s” to maintain patient care and daily clinic operation. Collaborate with clinic leadership to ensure equipment and supplies are readily available for patient care. Monitor supply and medication inventory. Ensure no medications or supplies are expired. Report and discard expired per policy. Alert specific personnel mentioned above when supplies are needed. We are seeking a dedicated Afterhours RN Care Manager to provide high-quality, compassionate hospice care outside standard business hours. This role ensures seamless coverage, continuity of care, and strong clinical management for patients and families during nights, weekends, and on-call hours. About the Role The Afterhours Care Manager RN is responsible for assessing and coordinating patient and family needs during after-hours shifts and maintaining primary clinical oversight for assigned patients. This includes routine visits, admissions, death visits, urgent after-hours calls, and any visit assigned per the patient’s plan of care. This role serves as an active member of the Interdisciplinary Group (IDG) to ensure coordination between physicians, families, and the hospice team. Clinical administrative responsibilities, workflow auditing, and survey readiness support are also part of this role. Support the IDG process through chart reviews, plan of care updates, and interdisciplinary communication. Develop and revise the Hospice Plan of Care in collaboration with physicians and IDG members. Educate patients and families on disease progression, symptom management, medications, and hospice philosophy. Communicate significant changes in patient condition promptly to hospice and attending physicians. Maintain accurate, timely clinical documentation including assessments, care plans, and notes. Assist with rescheduled, missed, or reassigned visit requests based on triage direction. Participate in calendar and scheduling modifications, including service codes and clinician assignments. Support administrative and compliance activities including workflow audits, survey readiness tasks, and documentation collection. Assist with TPE reviews, QAPI efforts, and regulatory compliance. Ensure infection control standards and universal precautions are consistently followed. Provide leadership and guidance to field staff; conduct supervisory visits when appropriate. Participate in required meetings, after-hours updates, and trainings. Maintain proficiency in pain and symptom management for hospice and end-of-life care. The Nurse Practitioner/Physician Assistant will perform physical exams, collect and document data, conduct diagnostic and therapeutic procedures, order and schedule laboratory studies and sub-specialty referrals, prescribe appropriate interventions and medications, and provide overall care coordination services. The PNP/PA will contribute to excellence in patient care, research, teaching, and provide leadership in the program. The Pediatric Nurse Practitioner (PNP)/Physician Assistant (PA) may also perform inpatient consults on infants from the pediatric wards who have discharge planning needs and will be following up in SICC or PCNC, such as those needing be discharged home with NG tubes. In this role, the Nurse Practitioner/Physician Assistant performs a thorough chart review, physical exam, and provides recommendations to the inpatient team to support the infant’s feeding or development and management related to their transition from hospital to home. Job Duties and Responsibilities Clinical Expert Collaborate with attending physician(s) and healthcare team to assess, diagnose, plan, set priorities and realistic outcomes, and evaluate the effectiveness and cost efficiency of patient care.
