Surveillance & Data Collection Perform routine surveillance for HAIs using standardized definitions. Collect, validate, and enter infection prevention data into NHSN and other required reporting systems. Monitor trends and assist in preparing reports for committees and leadership. Patient Safety Events, Risk Mitigation & Learning Partner with Risk Management to review patient safety events, near misses, and grievances; perform targeted chart reviews and case tracers. Facilitate or contribute to RCAs/After‑Action Reviews; develop corrective action plans (CAPs) and monitor effectiveness and sustainment. Lead proactive hazard surveillance and failure mode identification; embed feedback loops (huddles, visual controls, standard work audits). Performance Improvement & Education Facilitate Lean Model for Improvement projects (AIM, driver diagram, PDSA cycles) at the unit and service line level. Coach leaders, charge nurses, educators, and physicians on measure definitions, documentation, and bundle reliability tactics. Design and deliver adult‑learning education (tip sheets, micro‑learning, in‑services) based on identified performance gaps. Committee & Workgroup Facilitation; Stakeholder Engagement Lead or co‑lead relevant committees/work groups (e.g., Infection Prevention Work Group, Unit‑based councils, etc.). Prepare agendas, minutes, dashboards, and storyboards; track actions to closure and communicate progress. Job Description: Under the leadership of Chief Quality Officer/Director and advised by the Medical Director of Infection Prevention and the Vice President/Chief Medical Officer, as needed, the Infection Prevention Manager carries out the strategy and vision for the infection prevention program such that the program supports Community Hospital’s strategic plan, quality commitment, and values while complying with hospital policies/procedures and applicable laws and standards, including: The Joint Commission (TJC), California Code of Regulations, Title 22, Occupational Safety and Health Administration (OSHA), and the Centers for Disease Control and Prevention (CDC). The Infection Prevention Manager is responsible and accountable for overall management of the development and administration of the hospital’s infection prevention program to include policy development, program staff education, program coordination, assessing, planning, implementing, and evaluating all aspects of services delivered; ensuring quality programs, patient safety, and a level of customer service that strives to exceed internal and external customer expectations. The Infection Prevention Manager is also responsible for the daily operational oversight of the infection prevention program, including: surveillance, prevention, and control activities necessary to prevent and control infection, as well as managerial oversight of the infection prevention team. The Infection Prevention Manager develops and mentors a high-performing team for all areas of responsibility through practice of excellent employee relations, attention to employee needs (including fostering effective working relationships training, developing/coaching and evaluating), performance improvement initiatives, a collaborative environment, and initiating personnel actions, when necessary, in accordance with Human Resource policies and organization philosophy. The Infection Prevention Manager establishes and maintains effective working relationships with medical staff, organizational leaders, and other departments and fosters a collaborative environment with department leadership and staff in order to achieve department goals. Leadership of and/or participation in work groups or committees as required for success of the infection prevention program. Collaboration with Employee Health related to employee infections/immunizations/prophylaxis. Active participation in environmental inspections, continuing education and in-services. This position requires regular reporting to hospital Administration, the board of trustees, and keeping abreast of changing regulations. This position also required active participation during Joint Commission surveys and other inspections by regulatory agencies. Experience Previous experience in infection prevention and teaching adult learners preferred. Familiarity with Microsoft Office and an ability to learn and independently manage specialized infection control software. An ability to work well with people in order to facilitate changes that prevent or control infection is required Education Bachelor’s degree in nursing or BSN in progress achieving within one year of hire. Master’s Degree in Public Health, Science or Nursing preferred. Must be certified in infection prevention (CIC certification) by the Certification Board of Infection Control and Epidemiology, Inc. (CBIC). Licensure/Certifications State of California RN license required.
