Job Details The successful candidate will circulate and scrub cardiac diagnostic and Percutaneous Coronary Interventional (PCI) procedures performed in the Cath Lab. The RN will admit and recover pre and post cath patients. The RN will cross-train to hemodynamic monitoring and is responsible to perform hemostasis. Description The Medical Office Specialist offers clinical and medical office support to physicians and surgeons. Assists in patient care and administrative duties necessary to deliver quality patient care. Performs various duties as a member of the health team in the treatment and care of patient and their families and also assists with tasks to support the unit. Participates and supports the health team in the direct patient care under the supervision of an RN/LPN or MD. Description Title - Nurse Practitioner Manager, Palliative Care at Home - Mount Sinai Home Health - Full Time - Day Palliative Care at Home, part of Mount Sinai at Home, provides home based palliative symptom assessment, treatment and ongoing goals of care discussions for patients and families facing advanced illness. The full clinical team is comprised of CHWs, NPs, nurses, social workers and physicians. The team works in collaboration with patients’ healthcare teams to provide high quality evidence based complex palliative care and chronic care management and is designed to improve symptoms and QOL, reduce caregiver burden, reduce unnecessary inpatient utilization, and facilitate hospice when appropriate. Nurse practitioners will be responsible for the symptom management of chronically/terminally ill patients within an interdisciplinary team environment. This role will provide disease process and trajectory education to the team, patients and families. The NP will assess, order and interpret clinical testing, diagnose and treat symptoms related to a variety of chronic disease states to focus on palliative symptoms and advanced illness education.
