LPN Acute Care - Med Surg Unit 25

Overview LPNs take care of patients that have long term treatment needs. Often, these patients do not have a safe discharge plan to complete the treatment elsewhere and therefore continue to have a bed in the hospital. LPNs could have the opportunity to have hands on experience with wound care and IV antibiotics, among other long term care treatment. LPNs may often work beside acute care RNs but not directly in acute patient care areas. They work in collaboration with a C.N.A., charge RN, unit coordinator and physician. LPN staff with a variety of experience and backgrounds work on this unit. We strive to support learning and continuted education with all staff. Complex discharge units have medically stable but complex patients who are pending a safe discharge plan. The LPN must think critically, be able to problem solve and be ready to learn. Excellent communication with teammates and patients is essential. The ability to maintain compassion for this complex patient population is also essential. Staff on this unit value teamwork. Our physician partners work closely with this team to support in providing the best care posible for our patients. Responsibilities Assessment : Contributes to and communicates age appropriate admission and daily bio/psycho/social assessments. Collects, reports and records objective and subjective data in an accurate and timely manner. Observes the patient's condition or changes in condition. Overview Appointments to this job are always temporary, lasting no more than 6 months as part of a Medical Assisting training program. This is an on-site position, based in the Portland, Oregon metropolitan area. Candidates must reside in either Oregon or Washington state to be employed with Legacy Health. The Medical Assistant Extern Clinics is supervised at all times by a Lead Medical Assistant (MA), MA preceptor, or designee. This position is a learning opportunity for students currently enrolled in a Medical Assisting program, designed to develop skills and build clinical competency. LMG utilizes the “married state” preceptor/preceptee model. The Medical Assistant Extern Clinics is responsible for assisting with patient care, maintaining department/clinic efficiency, and other appropriate duties as assigned. This role participates as a member of the health care team in cooperation with and under the direction of a physician, while participating in daily clinic operations under the supervisor or manager. Responsibilities Prepares patients and rooms for exams and procedures. Performs basic laboratory testing, point of care testing, phlebotomy and other quality controls under supervision. Administers medications with direct supervision as directed by a physician or nurse practitioner. Assists physician/nurse practitioner with exams and procedures. Performs procedures with supervision until full competency has been documented by Lead, preceptor or designee. Accurately documents medical information in the patient records. Tracks and routes test results to physician(s) or nurse practitioner(s) for review, and file results in medical records per site operation practices. Communicates test results with patients when deemed appropriate by physician or nurse practitioner. Facilitates patient care activities, to include scheduling ancillary tests and reinforcing patient education, with direct supervision as appropriate and directed by physician nurse practitioner per site operation practices. Maintains adequate inventory of medical, office, and pharmaceutical supplies. Demonstrates and maintains professional behavior in accordance with established standards. Exercises timely, concise, and appropriate use of telephone to meet needs of external and internal customers. Performs basic computer tasks in using electronic health record proficiently while conducting daily documentation and communications involved in patient care. Overview Legacy Health offers an exciting growth opportunity to join a well-established team of collaborative and energetic Neonatal Nurse Practitioners (NNPs). This role provides the opportunity to practice across both Level III and Level IV NICU settings within the Legacy system. NNPs are integral to delivery attendance and the medical management of high-risk neonates and infants, including daily rounding on NNP service patients. Overview LPNs take care of patients that have long term treatment needs. Often, these patients do not have a safe discharge plan to complete the treatment elsewhere and therefore continue to have a bed in the hospital. Responsibilities Responsible for patient care within an assigned department/unit and participates as a member of the health care team in cooperation with and under the direction of the clinical manager or designee as defined by the relevant Standards and Scope of Practice for the Registered Nurse. The RN Staff Nurse assesses, analyzes, plans, implements and evaluates patient care following established patient care standards for Legacy Health System including those for patient assessment and transfer. Qualifications Education: As required by licensure. BSN or degree in progress strongly preferred. BSN may be required for specific assignments; experienced RNs without a BSN may be hired based on clinical experience with approval of the site CNO. Experience: One-year registered nursing experience or completion of either the Legacy Health RN Residency program or a nursing program that is equivalent in content, curriculum and duration. Additional clinical experience requirements may vary dependent upon the specific department in which the position is utilized. Minimum of one-year pediatric experience required. Critical care, PACU, or previous sedation experience is highly desirable. Strong IV skills preferred. Knowledge/Skills: Demonstrated ability of positive interpersonal and communication skills with colleagues, patients, families, physicians and the community. Keyboard skills and ability to navigate electronic systems applicable to job functions. Licensure Current applicable state RN license required. AHA BLS for Healthcare Providers. Responsibilities Completes history taking, physical examinations, diagnosis, and treatment of both chronic and episodic disorders. Assigned principal responsibility for primary health care of patients Plans and implements health care goals for patients. Responsible for the patient throughout the course of the treatment, provides the physical examination, the diagnosis, and the treatment plan. Refers patients to specialists for consultation or to specialized health resources for treatment but retains responsibility for clinical management as well as any subsequent modification. Collaborates to care for complex social and medical needs with health care team including but not limited to behavioral health clinicians, care management and social work. Qualifications Education: As required by licensure. Experience: Clinical experience requirements are dependent upon the specific position, and include a minimum of 2 years of experience in a related field.
