Responsibilities Assess and coordinate healthcare services for individuals, ensuring timely follow-up and communication with relevant clinical and residential staff Accompany and assist individuals with medical appointments in accordance with DDS guidelines Document all nursing interventions, health updates, and medication changes in Electronic Medical Records (EMR) and communication logs Monitor and maintain current medication and treatment orders, report side effects or adverse reactions as appropriate Review and support implementation of Individual Service Plans (ISPs), including self-medication, behavior plans, hygiene, and therapy programs Train residential staff on medication administration and health protocols Ensure up-to-date and compliant health records in line with DDS standards Communicate medical recommendations clearly to all program staff involved in the individual's care Maintain safety and monitor for health-related risks within the program Participate in staff meetings, mandatory trainings, and clinical supervision with the Health Care Director or program RN Perform medical interventions and additional responsibilities as assigned Uphold confidentiality, dignity, and respect for all individuals receiving services Foster a collaborative, supportive, and professional environment in alignment with Seven Hills Foundation values Qualifications Education & Experience: High school diploma, GED, or HiSET required. Licensed practical nurse required (LPN) 1-3 years of related experience preferred. CPR/First Aid and MAP certification required (paid training provided if needed). Skills and Knowledge: Strong communication and organizational abilities. Compassionate and patient approach to care. Commitment to fostering inclusion and independence. Ability to perform physical tasks, including lifting up to 50 pounds (or 200 pounds with assistance), and remain alert during shifts.
