Seven Hills Rhode Island is seeking a dedicated and compassionate Registered Nurse (RN) to join our Adult Day Health team. This role offers an opportunity to provide meaningful, person-centered care to individuals with intellectual and developmental disabilities, chronic health conditions, and complex medical needs in a supportive, community-based setting.

 

The Adult Day Health RN is responsible for assessing, coordinating, and delivering nursing care to program participants during daytime services. This position plays a critical role in supporting individualized health plans, monitoring participant wellness, administering medications and treatments, and responding to changes in condition.

Working collaboratively with an interdisciplinary team, the RN helps ensure safe program operations, regulatory compliance, and high-quality care that promotes independence, health, and overall quality of life.

Responsibilities

Clinical Care & Health Monitoring

  • Conduct nursing assessments and ongoing health monitoring of participants

  • Administer medications, treatments, and nursing interventions in accordance with physician orders and regulatory standards

  • Identify and respond to changes in medical condition, communicating updates to providers and team members

  • Maintain accurate and timely medical documentation, including MARs, assessments, and progress notes

Care Coordination & Collaboration

  • Collaborate with program staff, therapists, case managers, families, and healthcare providers to implement individualized care plans

  • Participate in interdisciplinary meetings and care coordination efforts

  • Support continuity of care and positive health outcomes for all participants

Program Oversight & Compliance

  • Provide nursing oversight for medication administration and health-related supports within the program

  • Ensure compliance with all state, federal, and local Adult Day Health regulations

  • Promote infection control practices and safe medication storage and handling

  • Monitor health-related trends and participate in quality assurance and improvement initiatives

Training & Support

  • Assist in training staff on medication administration, health monitoring, infection control, and emergency response procedures

  • Provide clinical guidance and support to program staff to strengthen care delivery
