Overview This individual is a clinically competent Cardio-Vascular Advance Practice Clinician (APC) will directly report to the Director of APCs with a matrix reporting structure to the Executive Director of the Cardiac and Vascular Service Line. S/he must be able to effectively deliver care through demonstrated expertise to the Cardio-Vascular patient. The principle duties and responsibilities include but are not limited to: performing pre-procedure assessments, Histories and Physicals, education and counseling of patients on cardiac and vascular diseases and treatments, responsible for tentative diagnosis, order and provide preliminary interpretation of lab studies, post procedure management of patient, discharge patient. Responsibilities Responsibilities Provides timely and thorough pre procedural work up on all Cardio-Vascular patients to include orders, History and Physical, review of consent and lab tests. Provides post procedural orders, discharge planning and education on all Cardio-Vascular patients. Provides direct patient care to Cardio-Vascular patients and evaluates patient outcomes. Participates with evaluation and eminence of closure devices, sheath pulls and arteriotomy sites. Performs orders and interprets results of diagnostic and lab tests on all pre and post cardio-vascular patients. Provides training and education to the CCL Staff, ED and inpatient units, staff and community as needed. Actively participates in mock codes and cardiac arrests on cardiac units. Actively participates in review of PI data and all PI activities associated with the cardio-vascular patient Assessment & Management Assesses for risks associated with the care of the acute and complex chronically ill patient including: medication side effects, immobility, impaired nutrition, fluid and electrolyte imbalance, immunocompetence, invasive interventions and diagnostic procedures. Orders and collects data using appropriate evidence-based assessment techniques, relevant supporting diagnostic information and diagnostic procedures where indicated. Assesses for risks associated with the care of the acute and complex chronically ill patient including: medication side effects, immobility, impaired nutrition, fluid and electrolyte imbalance, immunocompetence, invasive interventions and diagnostic procedures. Diagnosis Collaborates with interdisciplinary health care team in making diagnoses of acute and complex chronic conditions. Manages diagnostic tests through ordering, interpretation, performance, and supervision. Formulates differential diagnoses by priority. Diagnoses complications and orders appropriate interventions Formulates Plan of Care Identifies expected outcomes from diagnosis, and formulates and documents a plan of care to address complex acute and chronic health care needs. This position plays an integral role in the surgical team by collaborating with surgeons and operating room staff to ensure high-quality patient outcomes. The RNFA uses the full scope of the nursing process – assessment, planning, intervention, implementation, and evaluation – to deliver comprehensive surgical care and support throughout the perioperative continuum. Responsibilities Demonstrates competency in performance of scrub/surgical routines and aseptic technique--utilizes proper scrub, gown and gloving technique for self, surgeon, and assistant; ongoing monitoring of asepsis in all roles--scrub, circulator, and assistant; anticipates needs of surgeon; consistently recognizes sequence of procedures in all roles--scrub, circulator, and assistant; passes instruments correctly; appropriate review and preparation for assigned procedures. Understands and utilizes established guidelines for operation of autoclaves and steris units--performs daily A-Test/Biological tests as required; performs proper care, handling, and assembly of instruments for processing; ensures complete/thorough documentation of all flash and steris cycles as per protocol. Accepts personal accountability for proper handling, judicious use, and optimum care of all department supply, instrument, and equipment inventory. Utilizes the nursing process throughout the perioperative setting per patient assessment--collecting, interpreting, recording, and communicating pertinent data relevant to patient care in surgery; care planning--utilization of collected data to plan appropriately for intraoperative nursing care; implementing--provides direct patient care to meet physical and psychological needs; evaluating care--continuously evaluates patient response to nursing intervention and modifies care plan to achieve desired outcome.
