Overview Care Manager, RN - Capacity Coordination Center 4 x 10 hour shifts Days, with every other Weekend As part of a multi-disciplinary team, applies nursing knowledge and processes to facilitate patient transfers for inpatient hospitalization, between levels of care during inpatient hospitalization, and post-acute care discharge coordination. Processes require review of the medical record, extensive collaboration with hospital providers, and transportation teams using electronic and telephone communication. Capacity Coordination Center Nurses are responsible for providing a high level of customer service in a high stress, fast paced environment while maintaining flexibility and the ability to adapt, improvise and problem solve in rapidly changing conditions. How you make a difference As a Registered Nurse (RN), your primary responsibility is to provide patient care using the nursing process of assessment, planning, implementation, and evaluation. You will work under the supervision of Charge Nurse(s) to provide patient education and activities in accordance with your competencies and education. Key Responsibilities Assist in assessing and planning individual treatment programs, consulting with Charge Nurse(s) and other staff as necessary. Implement medical plans by administering medications, obtaining diagnostic tests, and assisting with medical or minor surgical procedures as needed. Count controlled substances and implement clinical and technical aspects of care in accordance with established policies, procedures, and protocols. Document nursing encounters using the SOAP form of charting as required by policy and attend mandatory staff meetings and training. Communicate information to nursing staff, physician, health care unit supervisory personnel or other staff as necessary. About this role The Discharge Planner coordinates healthcare discharge services, focusing on identifying community resources and scheduling follow-up appointments for patients nearing release. They provide both individual and group psychoeducational sessions to support patients’ transitions into the community. The role includes facilitating benefit applications and coordinating medication or residential needs post-release. Discharge Planners maintain contact with community providers and document all activities in health records. The position requires close collaboration with facility and parole staff to ensure continuity of care.
