Full Time Monday - Friday 8 am - 5 pm plus rotating call The Med- Surg ICCU (26) beds and the IMCU (18) beds are new state of the art intermediate critical care inpatient units that pride themselves in providing patient centered family care to adolescent, adult, and geriatric patient populations. The mission of these units will be to provide the utmost quality of care to patients and families along with physician collaboration to those patients that require a higher level of care but do not fit the care needs that an intensive care setting provides. Highlighting a care model that places focus on nurses delivering exceptional care and emphasizes a Primary Care Nursing model with nurse-to-patient ratios ideally 3:1. A nurse on our care team will be encouraged to grow and learn in a supportive team-oriented environment where each individual is satisfied, energized, and productive. Nurses will be dynamic in knowledge and skill as our patient’s service lines are diverse, and will support the delivery of care model that UMC embodies. Nurses serving in this unit can precipitate caring for patients with these conditions but certainly not limited to the following: post neurological trauma/intervention, post stroke with and without intervention, post/trauma surgical and orthopedic, acute GI bleed controlled with intervention and hemodynamically stable, DKA, advanced pulmonary patients requiring CPAP, BiPAP, high-flow oxygen, or mechanical ventilation whom are medically stable, continuous intermediate cardiac and hemodynamic monitoring, and patients requiring nursing interventions every 2 hours for (pulse ox, neuro checks, complex wound care, glucose monitoring, blood pressure); vital signs hourly for up to 4 hours. A multi-disciplinary approach to nursing care is utilized on the Medical ICCU and the IMCU. Registered professional nurse who is responsible for operations of nursing unit and ensures quality patient care, competent and responsible management and cost efficient and effective utilization of unit resources. Registered professional nurse who plans, supervises and evaluates patient care and nursing personnel. Relates effectively with other departments for optimal continuity of care. Has a working knowledge of organizational and nursing policies. Overview This Quality Assurance Registered Nurse will report directly to the Area Vice President and be responsible for overseeing and ensuring the highest quality of patient care, compliance with regulatory standards, and continuous performance improvement within the hospice program. Develop, implement, and oversee the Quality Assurance and Performance Improvement (QAPI) program for hospice services. Analyze clinical performance data to identify trends and implement process improvements. Conduct audits, chart reviews, and compliance assessments to ensure adherence to hospice regulations and standards. Facilitate quality improvement projects and initiatives based on identified areas of need. Monitor and report patient outcomes, infection control data, and adverse events. Ensure compliance with Medicare Conditions of Participation (CoPs), The Joint Commission (TJC), National Hospice and Palliative Care Organization (NHPCO), and state-specific hospice regulations. Stay updated on federal, state, and local regulations and ensure the organization’s policies and procedures align with regulatory changes. Prepare and assist in hospice surveys, inspections, and accreditation reviews. Collaborate with leadership to implement corrective action plans following compliance reviews. Oversee the design and implementation of validation procedures, test plans, and test cases Provide guidance and education to clinical staff on quality and regulatory requirements. Assist in the development and implementation of clinical best practices. Support ongoing training initiatives related to quality improvement, infection control, documentation standards, and patient safety. Ensure proper documentation practices to meet CMS (Centers for Medicare & Medicaid Services) and other regulatory requirements. Develop, write, and manage the execution of an overall software test strategy Implement and utilize QA tools Create and present risk assessments to identify potential risk Collaborate with IT to document practice care and documenting in accordance with clinical and quality initiatives Work with quality initiative teams to improve care delivery and documentation Work closely with nurses, physicians, social workers, chaplains, and hospice aides to promote high-quality patient-centered care. Act as a resource for clinical teams regarding hospice best practices and regulatory compliance. Participate in case reviews and interdisciplinary team (IDT) meetings to ensure quality goals are met. OVERNIGHT RN - Recovery | The Ranch at Dove Tree | Lubbock, Texas About the Job: PURPOSE STATEMENT: The Registered Nurse is responsible for providing nursing care to clients. Supervising care treatment and services of client care provided by LPNs, CNAs and other staff on assigned unit and shift. Developing, implementing, and evaluating an individual plan of care for each client. Assessment and re-assessment of clients. Communicating appropriate information with the interdisciplinary team. Reporting client concerns and changes in condition to supervisor and/or provider in a timely manner. Roles and Responsibilities: ESSENTIAL FUNCTIONS: Provides and coordinates care by assessing physical and behavioral health needs of client, develops and implements nursing care plans, maintains medical records and educates clients and their families about various physical and behavioral health conditions while ensuring client safety at all times. Assesses clients’ status on an ongoing basis and communicates finding to the providers timely. Ensures nursing treatment plans are specific to assessed needs and initial treatment plan is completed upon admission. Obtains provider’s orders (verbal or via telephone) accurately and documents appropriately; transcribes and reviews provider’s orders ensuring all required elements are present and appropriate per policy and procedures. Accurately administers medications, per ordered ,utilizing at least two client identifiers. Notifies providers of significant changes in status such as difficulties with detox, medical emergencies, pertinent lab results, or change in medical status regarding response to medications, etc. Assesses and documents effects of PRN medications. Maintains accountability of administration, recording, control and security of medications, including narcotics. Completes admission and discharge processes efficiently and accurately as directed. Confers regularly with treatment team (e.g., nurses, therapists, program managers, provider) regarding client care. Attends and participates in the unit activities such as therapy groups, family meetings as appropriate, staffing, unit meetings, etc. Provides education/explanation to clients, families, and co-workers regarding medication usage/maintenance, common side effects, client's rights, as well as other topics necessary for optimal client care. Monitors client acuity, census, staffing needs, and escalates coverage needs to supervisor/Charge RN. Ensures thorough, accurate documentation is completed for all clients on each shift and client assignment sheets are completed daily and tasks are carried out as assigned. Communicates thorough hand-off reports to oncoming staff and supervisors. EDUCATION/EXPERIENCE/SKILL REQUIREMENTS: Graduation from an approved/accredited school of nursing. Roles and Responsibilities: ESSENTIAL FUNCTIONS: Provides and coordinates care by assessing physical and behavioral health needs of client, develops and implements nursing care plans, maintains medical records and educates clients and their families about various physical and behavioral health conditions while ensuring client safety at all times. Assesses clients’ status on an ongoing basis and communicates finding to the providers timely. Ensures nursing treatment plans are specific to assessed needs and initial treatment plan is completed upon admission. Obtains provider’s orders (verbal or via telephone) accurately and documents appropriately; transcribes and reviews provider’s orders ensuring all required elements are present and appropriate per policy and procedures. Accurately administers medications, per ordered ,utilizing at least two client identifiers. Notifies providers of significant changes in status such as difficulties with detox, medical emergencies, pertinent lab results, or change in medical status regarding response to medications, etc. Assesses and documents effects of PRN medications. Maintains accountability of administration, recording, control and security of medications, including narcotics. Completes admission and discharge processes efficiently and accurately as directed. Confers regularly with treatment team (e.g., nurses, therapists, program managers, provider) regarding client care. Attends and participates in the unit activities such as therapy groups, family meetings as appropriate, staffing, unit meetings, etc. Provides education/explanation to clients, families, and co-workers regarding medication usage/maintenance, common side effects, client's rights, as well as other topics necessary for optimal client care. Monitors client acuity, census, staffing needs, and escalates coverage needs to supervisor/Charge RN. Ensures thorough, accurate documentation is completed for all clients on each shift and client assignment sheets are completed daily and tasks are carried out as assigned. Communicates thorough hand-off reports to oncoming staff and supervisors. EDUCATION/EXPERIENCE/SKILL REQUIREMENTS: Graduation from an approved/accredited school of nursing. One or more years’ nursing experience within clinical area of service provided by facility preferred. Experience in behavioral health/psychiatric/addiction recovery environment preferred. Knowledge of and experience with psychotropic medications and medication administration procedures. Ability to apply nursing standards and practice within an acute psychiatric setting LICENSES/DESIGNATIONS/CERTIFICATIONS: Valid current RN license from the state or from a compact state, where applicable. Medical Intermediate Critical Care Full-Time Nights 7pm-7am The Medical ICCU is a state of the art 26-bed intermediate critical care inpatient unit that prides itself in providing patient centered family care to adolescent, adult, and geriatric patient populations.
