Admissions Evaluator- Registered Nurse in person

Job Description The Manhattan Division of The New Jewish Home is searching for a full time in person Admissions Evaluator to join their team. Onsite location is at 120 West 106th Street, NY, NY 10025. The Duties And Responsibility Of The Admissions Evaluator Are Reviews PRI’s and all supporting clinical documentation for appropriateness to the facility. Serves as a liaison to hospitals through the metropolitan area and telephone consultations to confer with hospital staff and to seek out and evaluate potential applicants for admission to the facility. Completes all required clinical pre-admission assessment forms on all potential evaluated, gathering additional medical and social information as need for a thorough assessment. Begins the application process via telephone by disseminating information about The New Jewish Home programs; may gather psychosocial and financial information to complete applications with applicants and/or applicants. Documents all applicant clinical and psycho-social assessments. Submits monthly statistics and reports as requested by the Director of Admissions. Contacts Managed Care Companies to obtain pre-authorization; reviews level requirements to obtain appropriate level prior to admissions. Conducts tours of the facility with potential patients, families and or representatives. Interacts with HMO Case Managers prior to admissions, to review skilled needs and maximization of level. Weekend and Holiday admission coverage as needed. Performs other duties as required. Skills & Requirements JOB QUALIFICATIONS: Registered Nurse in the State of New York. MDS experience and PRI certification would be a plus. Previous experience in post acute admissions processes Acute Care experience preferred. Bilingual is a plus. Specialized Skills And Competencies Responds politely and helpfully to telephone and in-person requests for service consultations. Excellent writing and clinical assessment skills. Good working relationships with staff and referral services. Ability to multi-task and work accurately in a fast paced environment. Knowledge of Electronic Medical Record, Microsoft outlook, Word, and Excel. Required to speak and write in an understandable manner. Bilingual (English/Spanish) a plus. Other Skills And Competencies Ability to relate to adult and geriatric populations in a manner that respects their needs and capabilities. Consults assignment for special duties and sets proper priorities for following through on assignment. Is familiar with and takes proper action in meeting physical needs of resident.
