Conduct thorough patient assessments, including medical histories, physical examinations, and diagnostic evaluations, to determine appropriate rehabilitation goals and interventions. Monitor patient progress closely and adjust treatment plans as necessary to optimize functional outcomes and promote independence. Participate actively in multidisciplinary team meetings, case conferences, and educational activities to enhance clinical knowledge and skills. Maintain accurate and timely documentation of initial evaluations, treatment plans, progress notes, and discharge summaries in electronic medical records systems. Provide education and counseling to patients and their families regarding the rehabilitation process, expected outcomes, and post-discharge care. Position Summary: Performs a wide variety of patient care activities and supportive services under the supervision of a registered nurse. Evaluates nursing care actions and patient responses and documents each shift. Observes and responds appropriately to changes and complications in patient conditions. Report significant changes in patient condition to RN. Documents treatments and diagnostic tests with results and response. Documents in a clear, concise, accurate and legible manner. Observes assigned post-treatment/operative patients immediately upon return to floor and initiates appropriate nursing actions. Obtains RN signature for review and co-signature all entries. Participate in on-call rotating schedule and provide back-up assistance as requested Qualifications Must be a Registered Nurse licensed in the state of operation. Nursing experience required, with experience in Hospice or Home Health preferred. Must possess a sound knowledge of nursing principles, procedures and elements of patient family teaching.
