Responsibilities Completes patient assessments, formulates plan of care, oversees implementation of plan of care, evaluate and adjust plan of care utilizing critical thinking skills The performance of other nursing acts which are authorized or limited by the board, and which are consistent either with American Nursing’s Association Standards of Practice or with Standards of Practice established by nationally accepted organizations of registered nurses. Care, counsel health teaching and promotion, utilizes Teach Back methodology as affirmation of comprehension The administration of medication, intravenous therapy and treatment as prescribed by a LIP Supervision, delegation and follow-through to other personnel in the performance of activities relating to nursing care. Qualifications Education • Associate's Degree required. • Bachelor's Degree preferred. Licenses & Certifications • Registered Nurse required. • BLS certification required upon hire. • De-Escalation Training required within six (6) months of hire. • Driver's License required. Required Skills, Knowledge & Abilities • Responsible for preparation of and interpretation of client plans of care and systematic assessments/reassessments. • Responsible for performing nursing treatments, procedures, and total client care in accordance with established agency and nursing care standards. • Responsible for communicating pertinent client assessment/reassessment findings to physicians. • Responsible for initiating referrals to other healthcare organizations and community services based on identified client needs and physician orders. • Ability to travel to client residences in inclement weather and provide nursing care in varying environmental situations. • Strong clinical assessment, care planning, and critical thinking skills. • Ability to work independently while coordinating care with physicians, patients, families, and interdisciplinary healthcare teams. • Effective verbal and written communication skills. • Ability to prioritize patient care needs and manage multiple responsibilities in a home health environment. • Ability to maintain accurate and timely clinical documentation. • Ability to establish effective working relationships with patients, families, physicians, and community resources.
