Overview About the Role The Advanced Practice Nurse (APN) is a skilled health provider who through a written collaborative practice agreement with a physician is credentialed through the Medical Staff Office. Responsibilities What You’ll Do Collects comprehensive patient health data/history including past and present medical, surgical, family, and social history. Diagnostic tests and procedures relevant to the patient’s status are collected, interpreted, and documented at the Electronic Medical Record (EMR). Completes the medication reconciliation process by documenting the current list of medications (including over the counter and nutritional supplements) taken by patient at each visit. Performs and documents an appropriate comprehensive physical assessment of patients which will include a review of systems, evaluation of manifestations and toxicities related to medical conditions and anticoagulation management. In collaboration with the physician, critically analyzes the assessment data/diagnostic tests and determines a diagnosis. In addition, develops a comprehensive plan of care that includes interventions of treatments to attain outcomes. Evaluates patient progress in attaining expected treatment outcomes. Monitors for signs and symptoms related to disease progression/recurrence. Appropriately manages medical and anticoagulation management emergencies according to protocol/policy. Prescribes and/or orders medications. Obtains appropriate consents for surgery and procedures. Ensures completion of all needed pre-operative tests, forms, and documents. Documents assessments, prescribing, plan, modifications to plan, and evaluation of care in the EMR. Qualifications What You’ll Bring Required: Completion of an educational program which is designed and conducted to prepare graduates to practice as an NP/APN with a master’s degree in nursing and eligible for certification by a national certifying body. A current Registered/Professional Nurse license and registration in the State of New Jersey is required. Provides monitoring and continuity of care between departments/services; collaborates with Clinical Care Coordinators, Patient Navigators, and medical staff to coordinate continuity of care. Overview About the Role Participates as an essential member of the healthcare team by providing clinical lactation therapy for patients. Provides education and consultative services related to breastfeeding. Serves as a resource for physicians, nurses, and other healthcare providers in order to promote optimal patient care. Designs and provides patient and staff education programs. Responsibilities What You’ll Do Initiates and receives referrals from physicians, nursing staff, and patients. Develops, implements, and evaluates an educational plan for patients requiring lactation services. Assesses the educational needs of the breastfeeding mother and family. Identifies an individual plan to meet the breastfeeding mother’s needs. Assures that lactation services are provided in a timely manner. Provides direct patient care lactation service to patients. Performs in-patient consultations at the bedside and out-patient consultations, as necessary. Provides individual teaching and group classes to breastfeeding patients and families. Documents Breastfeeding Education and related Patient Assessment. Records teaching and progress on appropriate documentation. Submits accurate and timely documentation of billable services. Overview About the Role The primary purpose of the Critical Care Transport Team (CCTT) Staff Nurse is to safely move patients requiring PCU or ICU level of care within the University Hospital and Rutgers campus. The CCTT RN prioritizes transport requests based on acuity and workload logistics. The CCTT RN will utilize the Acuity Checklist for Critical Care Transport form to determine who will accompany the patient during transport and what equipment will be needed. The CCTT RN will require a thorough and complete SBAR (Situation, Background, Assessment and Recommendation) report, in person, from the primary care PCU/ICU RN managing the patient. The CCTT RN will complete their tasks with compassionate consideration of the patient’s requests and those of their family. The CCTT RN will utilize the Inter Transport Patient Data Form to chart the appropriate data as it applies to the individual patient’s circumstances. The CCTT Nurse takes immediate action in emergency situations and responds to patient needs during transport. Upon arrival, the CCTT RN will give a complete and thorough SBAR report to the receiving RN and will assist in the safe transfer of care. All transfers will be conducted with emphasis on preventing skin breakdown and the secure positioning of lines, tubes, and drains. The Professional Registered Nurse is a member of a designated care team and collaborates, coordinates and guides team members in the plan of transfer and safe transportation of their patient.
