What You Will Do: Provides direct care to patients under the supervision and delegation of the Nurse Practitioner, Physician Assistant or Physician within ProHealth Medical Group. What You Will Do: The RN Case Manager works collaboratively with patients, social workers, physicians, other practitioners, caregivers and community agencies in coordinating and overseeing the interdisciplinary plan of care for a patient in a home care/home hospice environment.
