Job Description Cordial Health is seeking a dedicated and compassionate Psychiatry Nurse Practitioner to join our team. Responsibilities: Conduct comprehensive psychiatric evaluations and assessments of elderly patients in nursing homes. Diagnose and develop individualized treatment plans based on the patient's mental health needs. Provide medication management and adjust treatment plans as necessary. Collaborate with interdisciplinary teams, including physicians, psychologists, and social workers, to ensure holistic patient care. Offer counseling and psychotherapy services to patients and their families. Educate patients and their families about mental health conditions, treatment options, and self-care strategies. Maintain accurate and up-to-date medical records in compliance with organizational and regulatory standards. Participate in clinical meetings, case conferences, and quality improvement initiatives. Requirements: Valid Nurse Practitioner license in the state of New York with specialization in Psychiatry (PMHNP). Ability to work independently and effectively manage a caseload of patients. Strong diagnostic skills and knowledge of psychiatric medications and treatment modalities. Excellent interpersonal and communication skills to establish rapport with patients, families, and interdisciplinary team members. RN Supervisor (Per Diem) The Paramount at Somers Rehabilitation and Nursing Center is seeking an RN Supervisor (Night Shift) to join our team. RN Supervisor (Per Diem) Responsibilities Include: Supervise all nursing care in the facility Assess residents as needed Document in residents medical record as needed Admit/readmit residents Ensure that a safe environment is maintained Assist unit nurses as needed Interact with facility physicians as needed Any other tasks as assigned by the DNS RN Supervisor (Per Diem) Qualifications/Requirements: Current NYS RN (Registered Nurse) license. Experience preferred. Working knowledge of EMR and proficient in documentation. Current CPR certification. Must be a compassionate and reliable team player.
