Under the direction of the RN, the Psychiatric Nursing Assistant (PNA) performs the activities related to the implementation of nursing care plans and the nursing standards, policies and procedures. The PNA is responsible for contributing to the development, evaluation and reassessment of care plans through observing and reporting changes in patient behavior and physical status. The PNA is also responsible for assisting in the maintenance of the safe and therapeutic environment of the unit. The individual is competent to care for patients in the following age groups: adult (ages 19 - 64) and geriatric (ages 65 +). Responsibilities POSITION SUMMARY To ensure in collaboration with the Nurse Manager that each neighborhood provide safe, appropriate and goal-directed care and to provide support and direct nursing staff. Attends management meetings as required, this includes Operations Meetings and other Management Training as scheduled Rounds on Neighborhoods for the purpose of obtaining clinical information on residents, to support and educate staff, to assist with family issues and endorses complete information to next shift. Promotes 24 x7 accurate, objective clinical and administrative reports. Follow-up on status of residents transferred to other facilities and reports follow up to oncoming shift, Team Leader and physician Communicates effectively with Center Departments regarding resident/family needs on tour of duty and documents communication in a clear and concise manner. Qualifications Education Requirements: Graduate of an accredited RN program B.S. N. (required); M.S.N. preferred RN in the State of New York BCLS certificate PALS certification preferred Completion of courses mandated for continuation of RN license, (ex. Infection Control course; Child Abuse course.) Experience Minimum of two years pediatric experience. Minimum of three years experience in supervisory role. Overview About the Role The primary purpose of the Staff Nurse (Infusion Center) position is to administer total patient care based on individualized needs of assigned patients and families in accordance with professional nursing standards. Completes the medication reconciliation process by documenting the current list of medications (including over the counter and nutritional supplements) taken by patient at each visit. Performs and documents an appropriate comprehensive physical assessment of patients which will include a review of systems, evaluation of manifestations and toxicities related to medical conditions and anticoagulation management. In collaboration with the physician, critically analyzes the assessment data/diagnostic tests and determines a diagnosis. In addition, develops a comprehensive plan of care that includes interventions of treatments to attain outcomes. Evaluates patient progress in attaining expected treatment outcomes. Monitors for signs and symptoms related to disease progression/recurrence. Appropriately manages medical and anticoagulation management emergencies according to protocol/policy. Prescribes and/or orders medications. Obtains appropriate consents for surgery and procedures. Ensures completion of all needed pre-operative tests, forms, and documents. Documents assessments, prescribing, plan, modifications to plan, and evaluation of care in the EMR. Qualifications What You’ll Bring Required: Completion of an educational program which is designed and conducted to prepare graduates to practice as an NP/APN with a master’s degree in nursing and eligible for certification by a national certifying body. A current Registered/Professional Nurse license and registration in the State of New Jersey is required. Evidence of excellent interpersonal skills, effective communication skills, and creative problem solving are essential. A minimum of two (2) years of clinical nursing experience and current BLS certification are required. To perform this job successfully, an individual must be able to perform in an expanded nursing role by providing specialty care to General Surgery patients in collaboration with the attending staff. How you make a difference The Nurse Practitioner (NP) under the supervision of the Site Medical Director and other site Physicians, offers a comprehensive range of medical services to incarcerated patients in accordance with established policy and state guidelines. They ensure necessary documentation of services is provided to the Site Medical Director or designee to monitor the provision of clinical services. Additionally, they communicate scheduling changes to the Medical Director and H.S.A. and make arrangements for coverage of medical services if unavailable for an extended period of time. Key Responsibilities Provides medical care to inmates, including conducting exams, chronic care clinics, and infirmary rounds. Maintains accurate and legible progress note documentation in SOAP format and adheres to formulary guidelines. Responds to emergencies and makes logical decisions regarding non-formulary drugs or off-site referrals. Supports existing policies and procedures for admission, transfer, and specialty consultant referrals. Maintains confidentiality and provides input into facility committees as requested. Qualifications & Requirements Education A graduate of an accredited school of nursing An advanced degree in an approved and accredited Nurse Practitioner program. Experience Prior medical/surgical and/or emergency/trauma experience and corrections experience is preferred. Must have the patience and tact to deal with the inmate patient population and ability to work effectively in the corrections environment. Scheduling flexibility is also required to be able to rotate hours and shifts, if needed, and to be called during emergency situations to provide coverage. Licenses/Certifications Must have and maintain current licensure as a Nurse Practitioner within the state of employment. Must have and maintain current CPR certification.
