JOB PURPOSE: Responsible for organization and assignment of duties for in home aides and collaboration with other disciplines and community agencies to ensure clients' needs are being met. Conduct quarterly supervisory visits to monitor the in home aide services being provided. KNOWLEDGE, SKILLS, ABILITIES: 1. Knowledge of medical, behavioral, and psychosocial problems and treatment. 2. Extensive knowledge of nursing principles, techniques, practices, theory, and application to complex casework, group work and community organization issues. 3. Knowledge of community development and group dynamics. 4. Knowledge of medical terminology, disease processes and their treatment as they relate to decisions regarding appropriate clinical interventions and appropriate therapies based on medical and psychological diagnosis. 5. Skills in stress management. 6. Ability to plan and execute diagnosis. 7. Precision in accurate recording in medical records, social assessments, and intervention techniques essential to deal with serious problems related to patient needs. 8. Computer and typing skills required. 9. Ability to deal with a very demanding and emotionally stressful job. Flexible hours are necessary. 10. Ability to work independently or with minimal supervision 11. Ability to work with difficult, uncooperative and non-compliant patients. KEY RESPONSIBILITES: 1. Information and referral MINIMUM EDUCATION REQUIRED: A RN who holds a current license in the state they are working in. Must completed and DMA certified training program within 90 days. trainee status candidates shall be directly supervised for 1 year by the supervisor. The supervisor must approve and sign all work produced by the trainee. An individual with a Bachelor's degree or who holds a nursing license as described above, without the number of years of experience, may be designated as an apprentice and shall be hired to act in the role of case manager. The supervisor of the case manager shall provide direct supervision and approve all waiver workflow documentation and tasks. MINIMUM EXPERIENCE REQUIRED: One year case management in homecare, long-term care, personal care or related work experience. Candidates with a higher educational background than listed shall also have one year of community experience (preferably case management in the health or medical field directly related to homecare, long-term care, or personal care.) MINIMUM LICENSURE/CERTIFICATION REQURED BY LAW: A valid nursing license. a valid drivers license and automobile liability insurance. Satisfactory results of a criminal history check, office of the inspector general Sanction Check. ADDITIONAL QUALIFICATIONS: Communicates well with clients and family members.
