We are looking for a compassionate Home Health Aide (HHA) or Certified Nursing Assistant (CNA ) in the Shamrock, TX area to provide assistance in daily living and personal care services in patients’ homes in accordance with a physician established care plan. This role is a 1099 contract opportunity, with care being provided at the patient’s residence. The current scheduling needs for this position will be established between the HHA/CNA, patient, and case manager. Shifts: 28hrs/week, flexible schedule, perfect for those looking for Part-Time or PRN work! Opportunities available for other patients in the area. Offers are contingent upon passing a background check. Responsibilities Executes physician prescribed plans of care and compliant documentation of care provided in system of record. Administers prescribed treatments, and therapies. Performs patient assessments, coordination of care. Performs various activities associated with daily living, communications, and monitoring of vitals while working at the patient’s home. Will be working with patients in a comfortable environment where the service is one-on-one. Communicates with family members, doctors, and other prescribed individuals regarding the health of patients. Performs other personal care services as necessary to meet the patient’s needs. Requirements Current CPR, First Aid, and 2-Step TB Test Prefer experience in a clinical care setting, home health preferred. Good interpersonal skills. Prolonged standing and walking required, with ability to lift up to 50 lbs. and move patients. Requires hand-eye coordination and manual dexterity. This is a full-time, project-based position with an anticipated duration of approximately three (3) years, dependent on actual completion. This is a full-time, project-based position with an anticipated duration of approximately three (3) years, dependent on actual completion. Full-time positions working 36–40 hours per week . Both 10-hour and 12-hour shifts are being considered. As we prepare to open this brand-new location, the schedule has not yet been finalized. We are open to exploring scheduling options based on candidate interest and availability, while ensuring the schedule supports the operational needs of the site. On-site housing is available, as needed, for new hires who live more than 30 miles from the location. What You’ll Do Conduct physical exams, preventive health assessments, fitness-for-duty evaluations, and return-to-work exams in accordance with established guidelines and regulatory requirements. Ensure compliance with applicable clinical and occupational health requirements, including OSHA, DOT, CDC, and other relevant standards. Perform clinical procedures such as injections, suturing, wound care, and infection management. Provide medically appropriate, evidence-based care to injured workers, supporting timely recovery and return to meaningful work. Develop individualized patient care plans, monitor progress, and ensure continuity of care. Implement and monitor transitional duty and return-to-work programs for injured employees. Order and interpret diagnostic tests to assess clinical conditions and guide treatment. Prescribe or recommend appropriate treatments, including medications and therapies. Educate patients on treatment plans, preventive care, health maintenance, and overall well-being. Collaborate with physicians, the Medical Director, healthcare teams, third-party administrators (TPAs), and client stakeholders to coordinate comprehensive patient care and case management. Partner with client stakeholders on health promotion programs, medical surveillance needs, ergonomics, process development, workplace safety, and injury-prevention initiatives. Refer patients to specialists or physicians when additional evaluation or treatment is needed. Maintain complete, accurate, and up-to-date patient health records and clinical documentation. Educate and coach nursing staff on clinical best practices. Perform other duties as assigned. What You’ll Bring Maintains current state RN licenses/certificates to practice. Maintains current Nurse Practitioner licenses/certificates to practice. Maintains State Prescriptive Authority, if applicable. Federal DEA license, matching each state of practice as applicable. State Narcotic/Controlled Substance license, if required in your state (Alabama, Connecticut, DC, Delaware, Hawaii, Idaho, Illinois, Indiana, Iowa, Louisiana, Maryland, Massachusetts, Michigan, Nevada, New Jersey, New Mexico, Oklahoma, Rhode Island, South Carolina, South Dakota, Utah, Wyoming, Puerto Rico), or New York ROPES registration, as applicable. Current national certification (ANCC or AANP required). Master’s Degree in Science of Nursing. If the Master’s Degree in Science certificate does not state Nursing, a copy of transcripts is required. Current certification with hands-on training in AHA, ARC, or equivalent Basic Life Support (BLS) for healthcare providers. Advanced Cardiac Life Support (ACLS) may also be required based on the contract scope of services. At least three years of clinical experience as a Nurse Practitioner in an ambulatory care, occupational health, family practice, or emergency department setting. Certification in occupational health (COHN/COHN-S/COHN-S/CM) preferred. Experience working in an occupational health setting is preferred. Experience performing site visits and collaborating with safety teams to identify and establish strategies to mitigate workplace hazards.
