Conduct office visits for routine and acute issues. Administer Annual wellness visits and health risk assessments, which require a holistic view of health and a focus on thoughtful, accurate, and specific documentation. Care coordination with other providers, specialists, testing facilities, and agencies. Population health leadership, in coordination with the Care Team (e.g., making sure all eligible females get their evidence-based breast cancer screening every 2 years) Assisting the care team with phone triage and outreach. Educating patients on their health conditions, care plans, and treatments. Participating in Oak Street Health promotional activities. Conducting home visits as needed. Other duties, as assigned. This role reports to the Center Medical Director and works closely with operational leadership. Key Responsibilities: Supervise, manage, and evaluate all nursing staff (RNs, LPNs, CNAs) Oversee daily clinical operations and ensure delivery of quality patient care Develop, implement, and monitor nursing policies and procedures in compliance with regulatory guidelines Direct nursing care planning, admissions, discharges, and care transitions Collaborate with medical staff, interdisciplinary teams, and other departments to support resident outcomes Ensure accurate documentation, charting, and record-keeping practices Monitor staffing needs, scheduling, and retention strategies Participate in facility quality assurance and performance improvement initiatives Provide training, mentorship, and performance evaluations for nursing personnel Monitor budgets and control nursing department expenditures Serve as a liaison between nursing staff and facility leadership Qualifications: Current RN license in the state of North Carolina (or eligible for reciprocity) Minimum 3–5 years of nursing leadership experience in a skilled nursing or long-term care setting Proven experience as a Director of Nursing, Assistant DON, or Nurse Manager Strong understanding of nursing regulations, clinical standards, and compliance requirements Excellent leadership, communication, and interpersonal skills Ability to build and lead an effective team while maintaining high staff morale Proficient in care planning, clinical systems, and EMR documentation CPR certification required; additional certifications a plus Strong organizational and critical-thinking skills Why Join Us: At Piedmont Hills, we are resident-centered and quality-focused .
