This role is responsible for assessing patient needs, performing fitness tests, and developing customized exercise programs to improve mobility and aid recovery from injury, illness, or cardiovascular events. Key duties include providing patient education on disease entities, risk factors, and behavior modification while continuously monitoring patients during exercise sessions. The Cardiac Rehab Clinician tracks patient progress through detailed reports and maintains critical communication with the Cardiac Rehabilitation Medical Director and primary physicians regarding clinical changes. Additionally, the role requires the ability to respond quickly and accurately to sudden changes in a patient’s condition or response to treatment. PRN Nights, 7p-7a JOB SUMMARY Seeking an experienced Emergency nurse to join the NTMC team and become a part of a dedicated team in North Texas rural medicine. Provides professional nursing care within the Emergency Department; coordinates care planning with other disciplines. Provides care that meets the psychosocial, physical and general aspects of care. Determines priorities of care based on physical and psychosocial needs of patients, as well as factors influencing patient flow through the system. Communicates with ED physicians regarding changes in patient’s status, symptomatology and results of diagnostic studies. Responds quickly and accurately to changes in condition or response to treatment. SUMMARY OF ESSENTIAL JOB FUNCTIONS Ability to perform a head-to-toe assessment on all patients and reassessments as per policy. This includes neonate, pediatric, adolescent and geriatric patients and the general patient population. Uses triage process to ensure timely and appropriate care to patients. Accurately assigns triage categories. Ability to adequately assess and reassess pain. Utilizes appropriate pain management techniques. Educates the patient and family regarding pain management. Ability to monitor hemodynamic status of patient and correctly interpret the results. Demonstrates knowledge of cardiac monitoring; identifies dysrhythmias and treats appropriately. Ability to perform waived testing (point-of-care testing, glucometers and fecal occult blood) per Clinical Laboratory’s and the patient care unit’s policies and procedures. Maintains current knowledge of medications and their correct administration based on age of the patient and his/her clinical condition. Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/healthcare errors. Treats patients and their families with respect and dignity. Identifies and addresses psychosocial, cultural, ethnic and religious/spiritual needs of patients and their families. Demonstrates knowledge of the principles of growth and development over the lifespan and the skills necessary to provide age appropriate care to the patients served. Interacts professionally with patient and family and involves patient and family in the formation of the plan of care. Ensures an adequate stock of supplies and proper functioning of equipment. Assists in cost containment through the appropriate ordering and conserving of supplies and equipment. Cooperates with other personnel to achieve departmental objectives; and interdepartmental objectives. Responds promptly to changes in the workload, adjusting assignments as necessary. Performs other related duties as assigned or requested. Provides resource information to other staff. Demonstrates the ability to assist physicians with procedures. Orients new employees to the Emergency Department Participates in problem solving with other healthcare team members related to management of clinical care area. Participates in performance improvement activities. Works at maintaining a good rapport and a cooperative working relationship with physicians, ancillary staff and unit staff. THR Urine Drug Collector training (Maintained every 5 years and required prior to providing independent care) within 30 days of hire Required What You Will Do It is the role of the Concierge to guide consumers and their "secondary guests" through the entire urgent care visit to enhance the consumer experience by maximizing throughput and delivering clinical excellence. The Concierge is responsible for a variety of clinical functions, including but not limited to assisting the provider with care and treatment, laboratory testing, x-ray examinations, and other duties. The Concierge also performs patient registration, collection of payments, rooming, and patient discharge. All Concierges are expected to maintain competency and perform in all assigned positions on a rotating basis. This position is a vital part of the urgent care team that works under the direction of the Site Manager and works closely with the provider and other clinical staff. Must demonstrate discretion; good judgment; diplomacy; decision-making; and a customer service attitude. Collects identification and payor related documentation, conducts registration using EPIC workflow. Measures and documents vitals and communicates the resulting data to the consumer and provider, including temperature, pulse, oxygen saturation, respiration, blood pressure, height, weight, and pain level. Documents chief complaint, allergies, health history, and current medications accurately in EPIC for review by provider. Performs and documents laboratory testing and collections per provider orders and/or standard operating procedures, to include venipuncture with prior venipuncture competency check-off. Administers and documents oral and intramuscular medication using the appropriate medication verification workflow. If a medical assistant, then may perform injections with the appropriate competency checkoff Assists provider with procedures to include, but not limited to maintaining sterile field, laceration repair, I&D, and application of ortho-goods and assists with ortho-glass application. Performs diagnostic x-rays and documents completion of radiology examinations within the licensed scope of practice as ordered and verified by the provider. Performs x-rays following established protocols and utilizes appropriate exposure factors for care and safety. Ensures images and applicable communication are transferred to radiology group for overread and assures all images have a report on file in the medical record. Behave in a professional and empathetic manner to build trust, comfort, understanding, and relationships with consumers and secondary guest(s) and in-line with patient experience standards. Welcomes the consumer and secondary guest(s) to the center (including eye contact, smile, greeting, and personal introduction) and inquires about the purpose and nature of the visit. Recognizes and notifies provider immediately of emergent patients, performs BLS, provides oxygen administration, and assists provider with emergency care. Schedules primary care referrals for consumers Maintains office supplies, medical supplies, and medications at appropriate par levels as assigned. JOB SUMMARY Provides professional nursing care within the Intensive Care Unit; coordinates care planning with other disciplines. Provides care the meets the psychosocial, physical and general aspects of care. Determines the priorities of care based on physical and psychosocial needs of patients, as well as factors influencing patient flow through the system. Communicates with physicians regarding changes in patient’s status, symptomatology and results of diagnostic studies. Responds quickly and accurately to changes in condition or response to treatment. SUMMARY OF ESSENTIAL JOB FUNCTIONS Provides direct care to the ICU patients. Ability to perform a head-to-toe assessment on all patients and reassessments as per policy. This includes pediatric, adolescent and geriatric patients and the general patient population. Ability to adequately assess and reassess pain. Utilizes appropriate pain management techniques. Educates the patient and family regarding pain management. Ability to monitor hemodynamic status of patient and correctly interpret results. Demonstrates knowledge of cardiac monitoring; identifies dysrhythmias and treats appropriately. Ability to perform waived testing (point-of-care testing, glucometers) per Clinical Laboratory’s and the patient care unit’s policies and procedures. Ability to interpret the results of waived tests (point-care-care testing, glucometers); takes appropriate action on waived test results. Maintains current knowledge of medications and their correct administration based on age of the patient and his/her clinical condition. Follows the six (6) medication rights and reduces the potential for medication errors. Formulates a teaching plan based upon identified learning needs and evaluates effectiveness of learning, family is included in teaching appropriate care. Demonstrates the ability to assist physicians with procedures and performs services requiring technical and manual skills. Interacts professionally with patient and family and involves patient and family in the formation of the plan of care. Ensures an adequate stock of supplies and proper functioning of equipment.
