Corewell Health is offering up to $1500 as a sign on bonus for this opportunity! About the unit We are a 64 bed Inpatient Rehab Unit that sees a variety of patients who need to work on both their functional and medical problems. We treat ortho patients recovering from hip fractures, complex knee replacement, back surgery with neurologic involvement, multi-trauma, and other orthopedic issues. We also see neurology patients recovering from stroke, brain and spinal cord tumors, Traumatic Brain Injuries, Parkinson’s, MS, and any cancer diagnosis with functional decline. We also have patients come to our unit who had amputations and other complex medical/surgical issues with functional decline. Patients participate in 15 hours of therapy across the 7-day week. Nursing continues the interventions practiced by the patients in therapy sessions, to continue safe mobility. For many of our patients, Rehab is the key to getting back home with their families. This is a very rewarding unit to work on, as we all get to take part in the progress patients make towards their goal of getting home. About this unit 5 North is a 38-bed unit caring for a wide variety of medical, surgical, and chronically ill patients. The unit specializes in the care of diabetes (insulin drips) and renal failure (Continuous Ambulatory Peritoneal Dialysis). The unit can accommodate patients who need telemetry (monitored remotely) and oximetry. The patient/nurse ratio is adjusted based on acuity. About the unit This 48-bed unit is dedicated to ortho/neuro/trauma. Some specific diagnoses’ are total knee, total hip, spinal fusions, and fractured hips. We are Stroke and Trauma certified and complete additional education every three months to better care for this population of patients. The Care Promoter 1, Market Float is an unlicensed medical professional cross-trained to perform routine administrative and clinical tasks to keep the medical centers running smoothly. The duties of Care Promoter Float vary from setting to setting, depending on the size, location and type. ESSENTIAL JOB DUTIES/RESPONSIBILITIES: Measures and records patient vital signs; records patient interview and medical history. Catalogs and communicates patient needs to the PCP, Clinician, Specialist and/or other clinical personnel. Provides health coaching to a defined group of patients to support healthy lifestyle choices. Follows up with coached patients via weekly calls.
