Part time Nights The Registered Nurse is responsible for performing a variety of duties to provide quality nursing care to residents and to coordinate total nursing care for residents; for participating with the Director of Nursing and the Unit Managers in establishing specific goals; for ensuring compliance with all operating policies and procedures and Michigan Department of Health & Human Services regulations and Federal Health Care Administration regulations; for communicating with and supervising appropriate personnel; for providing daily reports and other periodic reports; and for performing all the duties of a Certified Nurse Aide when required. Principal Duties and Responsibilities: Assess resident conditions to implement intervention and develop Care Plans. Accurately record resident observations in PCC and follow through on Care Plans. Assure facility medical records policies and procedures are adhered to. Make regular rounds of residents independently and with a physician; observe and evaluate resident symptoms, progress, and reactions to treatments and medications and implements interventions as necessary. Provide preventative, supportive, maintenance, and rehabilitative care directed to the physiologic and psychosocial needs and well-being of residents. Provide care to residents by performing a variety of treatments, including changing dressing, cleansing and/or irrigating wounds and incisions, performing Foley Catheter irrigations, IV therapy, and administering tube feedings, chest tubes and peritoneal dialysis. Request and assure supplies and medical equipment are available to perform care and treatments as required by the resident. Schedule appointments, physicians, therapy, and consults. Document in appointment book. Monitor and assist CNAs with personal resident care duties such as bathing, feeding and assisting with ambulation, and other routine accommodative services for resident comfort and well-being. Receive reports from outgoing nurse and make rounds to see each resident daily and as needed. Administer medications to residents according to the Public Health Code, Nursing Department Policies, and standards and procedures and as prescribed by the physician; notify appropriate clinical and/or nursing personnel of medication contraindications. Advise residents and/or responsible persons regarding changes in medications, treatment plans, and resident condition. Manage and document emergency care of residents, coordinating communications to physicians, responsible persons, DON and Administrator, hospital personnel, emergency transport, and other appropriate personnel. Perform comprehensive and focused nursing assessments in conjunction with changes in condition and scheduled programs. Admit, transfer, and discharge residents. Requests and assures supplies and equipment are available to perform care and treatment as required by the patient prior to or at the time of admission. Assure completion of weekly, periodic and discharge summaries. Perform quality improvement activities as assigned by the Director of Nursing. Monitor rate of accident/incidents occurrences and infection rate on the unit and develop suggestions for corrective action. Weekly skin assessment to monitor occurrence of pressure injuries or other skin conditions, providing the Director of Nursing written data on the number of occurrences. Report resident and staff complaint and all unusual occurrences to the Director of Nursing or Administrator and any allegation of Abuse/misappropriation or neglect to the administrator immediately. Participate in periodic regulatory surveys by the State of Michigan and participates in the preparation of the Plan of Correction response to an inspection survey. Complete and report resident injury to Unit Manager. Fully understands all aspects of residents’ rights, including the right to be free of restraints and free of abuse. Is responsible for promptly reporting to the charge nurse or administrator incidents or evidence of resident abuse or violation of residents’ rights. Know and support facility philosophy, standards, policies and procedures. Communicate with department heads on an as-needed basis to problem solve and plan for more effective patient care. Inform Director of Nursing when staffing requirements are not met. Present a professional appearance and serve as unit role model. Maintain own professional requirements for continuing education and staff development by participating in work related seminars, etc. Promotes compliance with accident prevention procedures, safety rules, and safe work practices to prevent employee injury and illness and control worker’s compensation costs. Ensure equipment is used in a safe manner and report any maintenance concerns to the Environment Plant Manager. Adheres to the Nurse Practice Act. Attend staff meetings and in-services as requested. Perform other tasks as required. Required/Desired Qualifications: Education, Training, and Experience: Must possess a current Michigan license as a Registered Nurse. CPR certification required. Two years of professional nursing experience in the long-term care setting is preferred. Specific skills, knowledge, and abilities: Knowledge of current federal and state laws and regulations that apply to the practice of nursing in a long-term care setting. Has strong organizational and time management skills and the ability to prioritize responsibilities. Ability to apply mathematical concepts such as fractions, percentages, ratios, and proportions to practical situations. Basic computer knowledge is required. Other Special Requirements: Tolerate frequent exposure to blood, body tissues, and fluids with occasional exposure to hazardous materials and infectious diseases. As well as, frequent exposure to chemicals and latex, and plastic materials used for personal protective equipment. Manual dexterity required to operate modern office equipment. The CLA’s responsibilities for each participant are specified in the CLA Care Plan. The following statements are intended to describe the essential elements, functions, and requirements of the position. SPECIFIC DUTIES AND FUNCTIONS: The CLA plan of care is based on the initial and on-going assessments that are performed by the manager, assigned RNCM/clinical team, and lead CLA. The CLA assists in providing and/or maintaining optimal physical and emotional comforts to the participants (residents) living in the community setting while embracing the culture, mission, values, and the practicing Ten Principles at PACE SEMI. The CLA assists in creating a nurturing supportive environment while helping participants (residents) achieve maximum self-reliance and independence within the community living setting. The CLA performs the care or services that are outlined in the individual participant’s care plan; any care guidelines that are pertinent to the participant’s diagnosis are specified in the care plan. The CLA keeps the manager, lead CLA, and clinical team informed of the participant’s progress. Immediately reports all customer service complaints, all deviations from set schedule and appointments to supervisor /manager or lead CLA. The CLA responds to emergency situations in accordance with established policies and procedures. Works only within established scope of practice. At this time the participant’s satisfaction with the Care Plan and CLA is evaluated Driving is required within PACE SEMI catchment area The CLA must have reliable transportation available on a daily basis, a current valid driver’s license, must maintain an acceptable driving record, and provide proof of current automobile insurance The CLA must have the ability to transfer and assist with moving patients; frequent walking, bending, and lifting of forty (40) pounds or more may be needed in the performance of duties The CLA may be required to change participant assignments based on the changing acuity level, participant needs, and participant census. 
: Medication Technician Position Summary: FULL-TIME-AFTERNOON SHIFT Under the general direction of the Executive Director and Wellness Director, primary responsibilities include, but are not limited to, administer medications to the residents in a safe, accurate, and timely manner. Qualifications and Required Experience for Medication Technician: Excellent communication skills and a compassion for older adults. Must possess valid driver’s license. Must possess a valid Medication Technician certificate within the state of Florida 1st Shift: 6:00 a.m. - 2:00 p.m. 2nd Shift: 2:00 p.m. - 10:00 p.m. 3rd Shift: 10:00 p.m. - 6:00 a.m. Primary Responsibilities for Medication Technician: Administers medications according to the physician order on the medication administration record (MAR). Documents medications administered on the MAR in accordance with company policy. Checks MARs immediately after administration and prior to end of shift to ensure documentation is complete and accurate. Conducts change of shift report with oncoming/outgoing shift personnel. Maintains security of medication cart and narcotic keys during entire shift. Conduct narcotic count at the beginning and end of every shift in accordance with company policy and procedure. Provides all necessary care and services to assigned residents and assists other caregivers as needed. Works collaboratively with peers and other team members. Provides assistance with Activities of Daily living such as with bathing, dressing, personal hygiene (includes shaving, oral care, nail care, hair care, foot care), toileting, eating and ambulation/mobility. Maintains professional demeanor at all times when interacting with residents and families. Reports any suspected or witnessed instances of verbal, mental, or physical abuse to licensed staff immediately. Utilizes safe transfer techniques when assisting residents with mobility. Participates in and supports the resident-centered activities program. Care for sensory enhancement devices such as eyeglasses and hearing aids. Completes housekeeping tasks such as bed-making, linen changes, vacuuming, emptying trash, and cleaning bathroom. Washing residents’ laundry as needed. Escorts residents and coordinates internal transportation needs. Assists residents with personal correspondence/telephone usage when requested. Observes and reports changes in residents’ physical condition and cognitive/emotional status to Wellness Director or charge nurse, as needed. Conducts room checks and resident rounds. Monitors for environmental safety hazards. Assists in a variety of tasks related to dining (escort residents to dining room, assist with set up and clean tables, serve meals, bus tables, etc.). Assists residents without swallowing difficulty with eating. Responds to emergencies in a prompt and calm manner. Immediately reports such emergencies to the charge nurse. Attends and participates in staff meetings and mandatory in-services. Documents provision of services on company forms. Uses tactful, diplomatic communication techniques in potentially sensitive or emotionally charged situations. Follows-up with appropriate staff, residents, or other individuals regarding reported complaints, problems and concerns. Acts as ambassador and public relations representative to guests and other off-campus visitors. Communicates with physicians and third party specialists, such as homecare providers, labs and hospice professionals. Copies special paperwork or forms. Demonstrates an appreciation of the heritage, values, and wisdom of the residents and an understanding of the aging process. Collaborates with associates in other departments to assure necessary care and services are provided. Participates in projects or committees as assigned. Performs related duties as assigned. Observes patients and reports any changes in patient conditions to the PCP, Clinician, Specialist and/or other clinical personnel. Perform non-critical medical and therapeutic procedures based on medical instructions. Collects phlebotomy and other lab specimens; performs basic waived lab tests. Prepares and administers medications; changes dressings, applies bandages, removes sutures and other first aid procedures; uses CPR skills when necessary, all under physician supervision. Maintains supplies, equipment, stocks and sterilizes instruments; practices OSHA safety standards; performs accurate, legal, and ethical documentation at all times. Processes patient phone messages, returning calls and routing them to other team members as appropriate; calls patients to obtain and relay pertinent information for the physician.
