Job Duties Effectively identifies, evaluates, and addresses acute and chronic problems as well as disease prevention and health promotion issues of the patients while administering quality patient care. Works in an independent and interdependent relationship with the physician, which allows for consultation, collaboration, or referral. Responsible for initial consultation and/or follow up diagnosis and treatment of acute and chronic healthcare issues of clinic patients. Prepares cardiac clearance letters to referring physicians, for transplant and pre-operative clearance.
