Required Qualifications Successful completion of an advanced practice Nurse Education Program Nurse Practitioner Certification as Adult, Geriatric, or Family NP Current Licensure through the Louisiana Board of Nurse Examiners or the ability to obtain licensure prior to employment CPR certification 2+ years of post-graduate experience working as a Nurse Practitioner Valid DEA license or the ability to obtain prior to employment Clinical knowledge necessary to make recommendations concerning the clinical assessment, evaluation and management of members Computer literate and experience with Electronic Medical Records Preferred Qualifications Experience in Gerontology, Internal Medicine, and/or Family Medicine Experience in a managed care and/or ambulatory environment Urgent care experience Compensation for this specialty generally ranges from $109,500 to $164,000. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. Growing together As part of the patient centered medical home team, the RN Care Manager engages in a collaborative process which assesses, plans, implements, coordinates, monitors, and evaluates options and services to meet a patient's health needs through communication and available resources to promote quality, cost-effective outcomes. The RN Care Manager collaborates with clinical teams in various health care settings in support of the patient, to include acute care, skilled nursing care setting, long term acute care setting, rehab facilities, custodial care, ambulatory care settings, and the patient's home. The RN Care Manager will facilitate the coordination of care resulting in more efficient utilization of health care resources, decreased hospital readmissions and improved health outcomes. THIS IS A HYBRID ROLE. YOU WILL WORK THREE DAYS IN THE CLINIC AND TWO DAYS IN A VIRTUAL CAPACITY. *** Primary Responsibilities Discharge Planning: Works with facility navigator to coordinate DC process and transition needs based on patient's diagnoses and status Care Coordination and Patient Support: Collaboration with the entire clinic team to proactively manage high risk patients by ensuring appropriate: Frequency of PCP visits, Specialist referrals, Testing/screenings Medication Management to include medication reconciliation post discharge and medication adherence Chronic disease education Coordinate home visits when necessary to assess patient's needs Providing self-management support to patients in order to improve their health outcomes Utilization Management: Review Home Health recertification for necessity Assessing appropriate use of services: Emergency room, admissions and re-admissions Review utilization reports to determine possible interventions/care coordination Clinic Team Communication: Leads daily/weekly/monthly discussions where patients' goals, outcomes, interventions, and recommendations for improvement are discussed with members of the patient's care team Clinic Support: Assist in Clinic Daily Duties as needed You'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in. Under general supervision, the Medical Assistant is responsible for providing patient care services, determining medical conditions, and preparing patients for examination and treatment under the direction of the Provider.
