Job Summary: Essential Duties and Responsibilities Supports and promotes the mission and values of Covenant Health Ministry. Renders professional nursing care to patients within an assigned unit of the hospital and carries out orders placed by provider responsible for the patient’s medical care. Provides comprehensive clinical documentation including care plans each shift Seeks feedback regarding practice and takes actions as appropriate. Communicates patient information appropriate for continuity and safe care through Bedside shift report, Multidisciplinary rounds, and Hourly Rounding (AIDET) Compliant in achieving unit specific competencies and requirements Completes all net learning modules within timeframe identified. Helps to promote a culture of well-being (ex. decreasing alarm fatigue). Promotes a positive culture/atmosphere within the department each shift. Provides a safe and secure environment for patients, visitors, and staff by following established procedures and protocols Identifies and assists in correction of patient care safety issues. Works within guidelines of Just Culture Environment to promote error reporting and adoption of safe practices. Assures issues are addressed through appropriate use of the chain of command Participates in Daily unit-based safety Huddles Knowledge and understanding of the organization's over-all strategic plan Remains up to date with unit-based nurse sensitive indicators (such as CAUTI, CLABSI, CDIFF, HAPU, FALLS, HH and VAE). Actively participates in quality initiatives and performance improvement to improve patient outcomes. Collaborates with nurses in other department/units to draw upon expertise in other clinical areas/specialties Seeks opportunities to assist all team members Develops and implements performance goals based on self-assessment and performance feedback Assures patient education materials are culturally competent Serves as preceptor for new nurses Participates in DNV survey readiness. About the Job: The Invasive Cardiology Nurse provides care to all age groups referred with various systems abnormalities. He/She assesses/evaluates patients and families in a collaborative manner. The Invasive Cardiology Nurse identifies, facilitates, and evaluates outcomes of nursing care for an individual patient or group of patients utilizing the nursing process. He/She coordinates all aspects of and provides technical/medical expertise to medical staff for all adult and pediatric diagnosis interventional cardiology procedures performed within their section. He/She documents relevant patient information in writing on all patients undergoing cardiology procedures in accordance with standards of documentation. The Invasive Cardiology Nurse works under the general supervision and is accountable to the RN Clinical Manager for nursing practice. Radiation Therapy Schedules, tracks and assures timeliness of diagnostic testing. Obtaining insurance authorization for diagnostic testing. Schedules all follow up clinic appointments. Reviews patient’s charts to ensure all necessary information is obtained prior to patient visit. Obtains an updated medication list for all follow-up patients. Coordinates needed services/referrals for follow up patients. Tracks patients missed appointments, and notification to patient to reschedule and sending a certified notice for repeated missed appointments, per protocol. Assists physicians with patient physical exams/procedures/preparation of needed equipment. Ensures all exam rooms are adequately stocked and cleaned. Checking for and replacement of any expired items. Responsible for lined inventory and distribution within the department. Coordinates weekly treatment visits for physicians and RNs. Orders all departmental needed clinical supplies in the Lawson system. Tracks supply of bulk stores and MDS items and makes recommendations to adjust par levels. Telesitter Maintains constant visual observation of the patients receiving remote visual monitoring. Participates in the hand-off, during shift change or when admitting new patients to the system, of pertinent information/behavior about patients being monitored. Ensures another telesitter in place before leaving monitoring station. Verbally redirects patients when necessary. Provides patients with explanations as necessary, but does not counsel or provide opinions. Who You Are: Current Nursing Assistant License in the State of New Hampshire required. BLS authorization preferred or obtained within 90 days of date of hire. High school diploma or equivalent preferred.
