Foundation Medical Partners, our multi-specialty group, spans 70+ practices across southern New Hampshire and northern Massachusetts, providing coordinated, patient-centered care to thousands each year. 4 East is a 31-bed cardiac telemetry unit serving a patient population of cardiac, catherization recovery, stroke, pulmonary embolism, diabetic and endocrine disorders, neurological and renal disorders. About the Job: As a member of the Patient Care Services team, the 4 East Registered Nurse provides comprehensive and compassionate nursing care to adolescent and adult patients with a variety of medical and surgical diagnoses with a specialization in cardiac patients, cardiac invasive procedures and telemetry monitoring. The registered nurse successfully completes a competency based orientation program to become proficient in serving the 4 East patient population. What You’ll Do: Assess and implements priority-based nursing care and establish goals to meet the patient’s needs. Integrate patient values, best practice, and evidence into patient care. Engage patients and/or families in the care delivery. Function competently within own scope of practice. Utilize a communication style/practice to meet the needs of the team and/or the situation. This unit also provides a high level of care to our respiratory, diabetic, and endocrine disorder patients, as well as those patients with neurological, renal, vascular, infectious disease, and other medical conditions. Our 4 East Staff members also support dialysis patients with their pre and post-dialysis care. These patients are admitted as inpatients, outpatients, or observation patients and are predominantly adults, though some older pediatric patients may be admitted to the unit based on their care needs. About the Job: Clinical Assistants (LNAs) are a combination of Health Unit Coordinators and Licensed Nursing Assistants. As a member of the Patient Care Services team, Clinical Assistants support in patient care and coordinate administrative functions of assigned patient care areas and facilitate patient flow throughout the department. What You’ll Do: ADL’s and patient care assistance Maintain a safe patient care environment Prepare and maintain patient records with appropriate data elements Collect and document basic patient care information as delegated Who You Are: Completed LNA program & active licensure in the state of NH Basic medical terminology. About the Job The Intravenous Specialist assesses and evaluates patients’ health status related to vascular access and IV therapy needs and develops individualized plans of care. This role serves as a clinical expert in vascular access, utilizing advanced vein visualization and ultrasound technology to place peripheral IVs, midlines, and PICC lines. The Intravenous Specialist collaborates closely with clinical staff, provides education and training, and supports high-quality patient care across the facility. What You’ll Do Operate ultrasound technology for successful midline and PICC line placement. Assess patient vascular access needs and prioritize care accordingly. Utilize vein visualization technology to support vascular access procedures. Maintain expert-level peripheral IV insertion skills. Function independently while managing patient and departmental priorities. Utilize facility information technology systems effectively for documentation. Communicate clearly and professionally with patients, families, physicians, and clinical staff. Educate and instruct clinical staff on vascular access best practices and techniques. Collaborate with multidisciplinary teams to support safe and effective IV therapy. Who You Are / Requirements Current New Hampshire licensure as a Registered Nurse (RN). Certification by the Vascular Access Certification Corporation (VACC) within one year of hire. Minimum of three (3) years of related clinical experience. Demonstrated expertise in vascular access and intravenous therapy practices. Strong communication, teaching, and collaboration skills. Ability to work independently and effectively prioritize patient care needs.
