Provides and coordinates high‑quality clinical care; supervises and assigns tasks to home health aides and LPN/LVNs in compliance with state and federal regulations, including required supervisory visits. Accurately documents all patient care per policy and payer requirements; communicates timely with physicians, patients, families, and agency staff to ensure coordinated, effective care. Participates in orientation and precepting of new staff; contributes to performance improvement, survey readiness, patient safety initiatives, and reducing avoidable hospitalizations.
