MDS Registered Nurse or Licensed Practical Nurse

The MDS / Care Plan Coordinator is responsible for the accurate and timely completion of all Medicare/Medicaid case-mix documents in order to assure appropriate reimbursement for care and services provided within the Facility. Additional Details We are looking for a part time LPN to join our clinical team at Kane County Jail. This position is 16 hours a week, works 3p - 11:30p and rotates every other weekend. Qualifications & Requirements Education Graduate from an accredited school of nursing.
