General Summary The Office LPN assists the physician or mid-level provider by providing assessment’s, treatment, testing, and education for the patients of the practice. Essential Duties and Responsibilities Follows established clinic policy regarding patient safety measures such as infection prevention, safe patient handling and movement, critical lab notification, etc. to ensure that patient care is provided in an optimally safe environment. Administers medications (oral, IM, and SC) as ordered. Assists the provider with procedures, maintaining sterile field and managing instrument trays. Assists with examination as requested within scope of license. Phone triage. Acts as a general liaison between physician/APP and patient. Calls patients with normal/abnormal test results, answer’s questions and assisting physician with return calls. Documents chief complaint/visit reason and details according to provider preference. Escorts patients to exam room and obtains patient height, weight, and a full set of vital signs accurately. Initiates, reviews, and updates patient's medication and summary list according to provider preference. Performs point of care lab testing and obtains blood samples and other specimens for lab testing as ordered. Informs and sets up patients and/or guardian for My Chart at the time of service and informs and educates patients regarding the benefits of signing up for My Chart. Provides patient teaching and supportive services as needed. Provides treatments as ordered. Schedules diagnostic and therapeutic procedures, obtain’ s precertification and authorization as required by payers and ordered by provider. Trains LPN and MA in the clinic role. Meets commitments, works independently, accepts accountability, handles change, sets personal standards, stays focused under pressure, is a team player, and meets attendance/punctuality requirements. Education Graduate of an accredited LPN program and approved IV therapy course. Work Experience Nursing or other health care experience preferred but not required. Certification/License Current LPN license in the state of Missouri required. American Heart Association Basic Life Support required. Mental/Physical Requirements Considerable mental concentration required. Lifting up to 35 lbs., turning activities and nearly constant walking required. Standing, turning, carrying, pushing, pulling, stooping, crouching, twisting, and reaching. Working Conditions Frequent exposure to infectious disease and hostile persons at times. Frequent exposure to communicable disease via blood and other body fluids. Minimal physical discomfort. LPN (Licensed Practical Nurse) Job Description: Works under direct supervision using the Nurse Practice Act, Company standards, Policies and Procedures and nursing judgment. Assesses patients/residents and makes observations and report pertinent information related to the care of the patient/resident. Implements the patient/resident plan of care and evaluates the patient/resident response. Directs and supervises care given by other nursing personnel in selected situations. Maintains knowledge of necessary documentation requirements. Maintains knowledge of equipment set-up, maintenance and use, i.e. monitors, infusion devices, drain devices, etc. Maintains confidentiality and patient/resident rights, regarding all patient/resident personnel information. Provides patient/resident/family/caregiver education as directed. Conducts self in a professional manner in compliance with unit and facility polices. Works rotating shifts, holidays and weekends as scheduled. Initiates emergency support measure (i.e., CPR, protects patient/residents from injury). LPN (Licensed Practical Nurse) Qualifications/Requirements: Current/valid Florida LPN (Licensed Practical Nurse) License Previous experience in long term care preferred. Current CPR card Ability to work independently or part of a group. Effectively communicate with others. A minimum of one (1) year nursing experience in a long-term or acute care setting preferred.
