Incorporates patient care goals established in the Plan of Care, into nursing visits, as evidenced by documentation on skilled nursing notes. Communicates significant findings, problems and changes in condition or environment to the RN Case Manager, the physician, clinical supervisor, and/or other personnel involved with patient care. Reports unsafe conditions to the appropriate RN Case Manager, clinical supervisor, and physician, as appropriate. Implements the plan for patient safety, using the patient, family, and community resources. Assists the patient in learning appropriate self-care techniques while delivering quality patient care. Provides those services required substantial and specialized nursing skills under the direction of a Registered Nurse or physician. Documents medication regimen and updates medication profile according to established procedures. Treats patients and caregivers in the highest and most effective manner. Implement all available actions to prevent avoidable hospitalizations and ER visits. Provides positive, supportive communication to physicians, patients, families, visitors, and agency personnel. Position Summary: Performs nursing procedures/treatments in the home within scope of practice. Works under the direction of the registered professional nurse. Essential Functions and Responsibilities: Performs a variety of basic nursing procedures under the direction of a registered nurse which may include, but are not limited to, wound care, wound irrigations, medication administration, catheterization, range of motion exercises and glucose monitoring. If the LPN is appropriately qualified, prepares and administers oral, rectal, subcutaneous, intramuscular, intradermal and topical medications to patients per policy and procedure. Observes patient response to medication, records unexpected reactions and reports them to the registered nurse. Identifies common patient problems, symptoms and reactions through monitoring Participates in the development, review and evaluation of the plan of care appropriate to the age of the patient in conjunction with the registered nurse. Demonstrates sensitivity to patient comfort and privacy and expresses interest in the patient progress. Provides for the physical and emotional comfort of the patient as well as a safe home environment. Required: Current licensure with the Michigan State Board of Nursing. Two years current nursing experience. Current CPR certification. Valid driver’s license, proof of valid auto insurance, and maintain a driving record in accordance with McLaren’s corporate driving policy and eligibility guidelines.
