Position Summary: The primary purpose of your job is to assess resident’s needs, develop an individualized care plan, administer nursing care, evaluate nursing care and supervise nursing assistants and other personnel in the delivery of nursing care in a SNF setting. Such supervision must be in accordance with current federal, state, and local standards, guidelines, and regulations that govern our community, and as may be required by the Director of Nursing Services or designee to ensure that the highest degree of quality care is maintained at all times. Follows all health, sanitary and infection control policies and maintains established standards of practice set forth by the community's administration and Nursing Policies and Procedures. Education/Qualifications: Must possess a current, unencumbered license to practice as a LPN within the state in which you work. Must possess a current CPR certification. The Opportunity The Licensed Caregiver – LPN/LVN is part of the Health & Wellness Team responsible for providing high-quality care to our residents under the supervision of registered nurses and healthcare professionals. What You’ll Do Detail daily reports to Nurse, or designated charge person, at end of shift Observe and report on residents’ health, escalating any problems efficiently and effectively Helping residents with personal hygiene, including bathing, grooming, and dressing Supporting residents with medication. Repositioning residents in bed and escorting them around the community Toileting, incontinent care Take vital signs, such as blood pressure, heart rate and pulse Provide emotional support and serve as a source of comfort for residents and their loved ones Recognize abnormal changes in body functioning and report to a supervisor Perform nursing assessments, including vital signs monitoring, physical examinations, and health status evaluations, to identify and address resident needs promptly. Provide wound care and other nursing interventions as directed by the registered nurse or healthcare provider, following applicable state regulations, established protocols and infection control practices. Responsibilities include customer service, safety, resident care, and communication. What You’ll Bring POSITION REQUIREMENTS / QUALIFICATIONS: Must have a valid, unrestricted nursing license in the state. Must be at least 18 years of age. Must pass State and Company criminal background/drug screens. Demonstrates effective time management skills. High school graduate or equivalent. Current CPR certification. Maintain CEU requirements. Read, write, speak and understand the English language. Make independent decisions when circumstances warrant such action. Interpret and implement written or oral policies, procedures, and instructions. Staff supervision Assessment and resident care skills; Medication management principles and practice; and Written and verbal communication skills.
