Communicates essential information to appropriate parties. 9. Makes rounds on assigned residents at least once every two hours. 10. Collaborates with Care Plan Coordinator to develop and maintain nursing care plans and update these plans on a regular basis and as the resident's condition changes. 11. Attends and participates in interdisciplinary resident care conferences as requested. 12. Assists the attending physician with rounds and special procedures as needed. 13. Participates in the yearly written evaluations of nursing assistants and documents accurately their strengths, weaknesses and the steps that need to be taken to improve their performance. 14. Assists Staff Development with orientation and on-the-job training for new nursing personnel as needed. 15. Collaborates with nursing supervisor in documenting disciplinary concerns for nursing assistants on designated shifts, including reprimands, oral and written warnings, and corrective action recommendations. 16.
