The Longevity Program combines a comprehensive geriatric assessment with chronic disease management, cognitive health, medication review and weight management, organized around five pillars of healthy aging: sleep, nutrition, movement, mental health and social connection. It works alongside a patient's existing care and never replaces their primary care provider. The role This is an in-person contract role. We're looking for nurse practitioners in the Connecticut shoreline (Groton-New Haven) area to lead longevity days at our partner senior living communities. Each day opens with a short educational talk for residents on healthy aging, using a presentation and speaker notes we provide. You then see the residents who sign up for one-on-one longevity visits. You'll be trained and supported by our Chief Medical Officer, a board-certified geriatrician who designed the program's clinical content. Your first site day is run alongside a member of our team. Schedule Site days are on weekdays. The presentation is the fixed part of the day. The rest depends on how many residents sign up. Functioning in lieu of or alongside of a physician, it is essential for an urgent care Advanced Practitioner to have excellent assessment skills in order to recognize more serious conditions and direct the patient to the emergency room for advanced care when necessary. Job Requirements Education Bachelor's Degree required for Physician Assistants Master's Degree required for Nurse Practitioners Work Experience One (1) year of full-time experience in Emergency Medicine or One (1) year of Urgent Care setting experience required. Will consider 2 years of Family Medicine experience. Required Licenses/Certifications National Certification Active state medical licensure Additional Knowledge, Skills and Abilities Work experience in an Urgent Care or Emergency Room setting preferred Applicable office procedures including but not limited to suturing, splinting, casting etc. Preliminary reads on EKGs and X-rays required.
