The Licensed Practical Nurse is responsible for the overall care of those we serve within the Nursing, Personal Care or Assisted Living facilities. Work to ensure that residents receive high quality care and provide oversight and direction to Caregiver/MedTech or C.N.A. staff member. Qualifications Licensed Practical Nurse qualifications include: Graduated from an accredited school of nursing and possesses a current license, in good standing, from the Commonwealth of Pennsylvania. A graduate nurse who has not yet obtained board results or a Nurse who does not yet have a Pennsylvania license must have a temporary practice permit required by the Pennsylvania State Board of Nursing. Qualifications Skilled Nurse from an Accredited program currently licensed and in good standing with the Pennsylvania State Board of Nursing. One to 2 years’ experience in nursing in an acute care setting or 1-year experience in home health, hospice or comparable experience is preferred. Responsibilities It is the responsibility of the Visit Nurse to coordinate and provide the highest level of hospice care to patients and families. Coordination of services include communicating and making referrals for home health aide/homemaker, chaplain, social worker, volunteer, and ancillary services, as well as ordering medications and Durable Medical Equipment · educate patient and family on the disease process and expectation related to the disease progression. · assist with CQI activities. · work as a team member at all times and uphold the Hospice Code of Ethics · provide care to manage patient symptoms. · utilize the medication formulary whenever possible for the management of patient symptoms. · reports to DON as patient condition changes, changes in the patient condition that require change in level of care. III. Orientation and Training Care Comfort Hospice will provide orientation and training, as necessary, to assist the Hospice Nurse in performing the responsibilities listed above. Training may be in-house or outside seminars. Evidence of orientation and training will become part of the personnel file. IV. On Call The Hospice Nurse will be on call according to scheduling. During the hours when on call, the Visit Nurse is expected to follow On Call Protocol and to respond to all pages within 20 minutes. V.
