Advanced Practice Provider (NP/PA) – Addiction Services & Ambulatory Program (ASAP) 0.75 FTE | Three 10-Hour Shifts per Week Join Our Team The University of New Mexico Hospital's Addiction Services and Ambulatory Program (ASAP) is seeking a compassionate and motivated Advanced Practice Provider (Nurse Practitioner preferred) to provide high-quality outpatient care to adults with substance use disorders and co-occurring behavioral health conditions. This position offers the opportunity to make a meaningful impact on the lives of individuals in recovery while working alongside an interdisciplinary team committed to evidence-based addiction treatment and patient-centered care. Position Summary The Advanced Practice Provider will deliver comprehensive addiction and behavioral health services for adult patients in an outpatient clinic and walk-in care setting. Responsibilities include conducting assessments, developing treatment plans, managing medications, providing follow-up care, and coordinating services that support recovery and long-term wellness Schedule 0.75 FTE Three (3) 10-hour shifts per week Day shift schedule No call responsibilities Primary Responsibilities Perform comprehensive psychiatric and substance use evaluations for adult patients Diagnose and treat substance use disorders and co-occurring mental health conditions Provide medication management, including medications for addiction treatment (MAT) Develop individualized treatment plans in collaboration with patients and the care team Monitor treatment progress and adjust care plans as clinically indicated Coordinate care with therapists, case managers, social workers, primary care providers, and community resources Document patient encounters in accordance with regulatory and organizational requirements Participate in quality improvement and patient safety initiatives Compensation Disclaimer Compensation for this role is based on a number of factors, including but not limited to experience, education, and other business and organizational considerations. Ensure adherence to Hospitals and departmental policies and procedures. Patient care assignment may include neonate, pediatric, adolescent, adult and geriatric age groups. Title: Oak Street Health New Graduate Nurse Practitioner Fellowship Location: Zuni Road - 5555 Zuni Rd SE Suite 11, Albuquerque, NM 87108 Fellowship Program Description: Start your NP career in the future of healthcare! Oak Street Health’s NP Fellowship in Adult Primary Care is in formal collaboration with the University of Michigan School of Nursing. The program is set in the revolutionary value-based care model, which rewards healthcare companies for the quality of their care, not the quantity. This 12-month paid fellowship will provide all the necessary skills, support, and learning required to become a highly confident and competent clinician for older adults. During the program, fellows spend time building their own patient panel, learning from a multitude of preceptors, and engaging in didactic content that aims to build on, not repeat, graduate training. All fellows commit to working at Oak Street for at least a year following program completion, but we hope they will spend their careers at Oak Street improving the health of communities across the country. The program has two concurrent cohorts starting in September and March of every year. The annual salary for this location is $105,000. Benefits: One-year paid program. Requirements: Family, adult, or adult-gerontology primary care nurse practitioners who graduated or will graduate from their programs between December 2024 and May 2026. Must currently reside in, or be willing to relocate to, the Fellowship program location for at least the next two years. Have not yet worked as an NP. Minimum of one year experience, within the last five years, working as an adult RN. Minimum GPA of 3.0 in NP program. Have passed, or in the process of, passing NP board examinations by 6/6/2026. Currently holds or will hold by 8/3/2026 an active, non-probationary state NP license. A genuine passion for adult primary care. A supportive attitude toward our patient population of older adults.
